Hey, D3DB.com (another nice website) have scanned the "armory" for deaths in hc, and especially deaths in inferno.
Here are top 10 reasons for deaths (and als how many of each)
I find it quite funny how its stuff like Walking Corpses, Skeletons and Risen that are the top killers. Theese are probably some of the weakest mobs, making me think that people often underestimate them.

I underestimated azmodan's (hell difficulty) fireball because on my softy it takes away 20% of my HP. It hit me and I was @ 1% hp running around screaming. I had to have been that low for about 20 seconds. Finally potted and lived. I'm now 60(2) :D. It was great because my buddy was with me in game and on ventrilo, telling me I was gonna die over and over.
Belial I understand but I also don't understand Azmodan being on there seeing as he's the easiest boss in the game.
It's obvious why act 1 mobs kill more players in Inferno than any other... most people don't get past act 1 Inferno in hardcore. If those are rares they are just as deadly as any other if you're not prepared.
Wasn't expecting those to be the top killing mobs either, i was expecting those suiciders from act 3, they killed my friend and allmost me!

I Agree with this, but not entirely. There are some insanely random spawns of suiciders around, like i mentioned with my mate. We were just strolling through hell Act 2, found a shipwreck "on the road to alcarnus" and clicked on a dude. Black rock ledger page pops up and takes our attention, second later 5-6 suiciders blast my mate. I didn't even get to react. He goes down even though he pops diamond skin! He had over 30k hp. That was an insanely unpredictable and random way to die!
By the way, where can you check what your character died from?

...Who the hell dies on Azmodan ever? Easiest attacks to avoid, and if you're playing HC you should have way more than enough health to take a hit
Lost my 60(17) Demon Hunter one-shot to Azmodan fireball. I had 800 fire res and 65k HP. I also thought i could take 1 hit so i didn't really bother to avoid it Guess what... Fatal mistake!
That kinda sucks. I guess this explains azmodan too, he is being underestimated just like carrion bats and shiz, and therefore people die on him.
IMO, the most dangerous HC killers are:
1. underestimation of danger / overestimation of own ability to survive
2. fatigue
Me (barbarian, wizard) and 2 of my friends (DH and monk) started playing HC Inferno like two weeks ago. We killed softcore Inferno before major difficulty nerfs, so I think it's safe to say we have some experience in this game. We thought that experience will help us evade the most common mistakes people make in HC.
Yesterday my lvl 52 DH friend decided to play solo a bit before going to work. He died fighting a pack of reflect + waller + desecrator spiders. The thing is we usualy stomp such packs into the ground without much problem when play in a group, so he thought it'll be just as easy solo and decided to Gloom-tank instead of kiting them carefully.
Later that day me (on my barb) and my monk friend were clearing Hell A2. We both use super-defensive builds with as much saves, buffs and "-% damage" debuffs as possible, so everything went smoothly. We were playing for like 2 hours non-stop. At some point I realized I totally lost focus and stopped paying any attention to what's going on. I stopped evading fire/arcane/plague crap and stopped watching my HP level - partly because everything was so easy, partly because I was tired after a long day at work. I guess the same thing happened to my friend, because he died recklessly face-tanking a pack of arcane + desecrator + something melee snakes right in the middle of a fire & arcane mess.
What this all tells me:
1. avoid playing HC when you can't pay full attention to the situation
2. make short breaks at least each 30-40 minutes - port to the town, get some tea/coffee, do something else for 5 minutes then get back
3. if you see something at least 1% nasty - start kiting towards the nearest portal through the clear zone
P.S. Yeah, I know I wrote a fucking book and I guess I said nothing new there. But I just wanted to share my thoughts on the matter and obviously I can't discuss it with my friends right now, since they're upset.
P.P.S. Well, at least stash and gold is not wasted and my wizard can AOE-train my friends' new DH and Monk back to 50+ quite fast, so life goes on.

2. Because (according to Blizzard's statistics) like 80% people die in A1 Normal before even encountering maniacs.
Most of those monsters are on the list because of their location in the game (Act 1). The statistics only count the gross number of deaths, and since the majority of HC characters don't make it out of Act 1, the numbers of deaths to Act 1 monsters are heavily skewed. Doesn't have anything to do with underestimating the foe.

And why all those people die in A1 if not because of underestimation? I believe you can't create a HC char until you kill softcore Normal mode at least once, so there's no way all those people die because they're completely inexperienced and don't know what they're doing.

You only need to get to level 10 (so pretty much kill the SK) to unlock HC. My friends and I did our first playthrough on HC all the way to 60 so we wouldn't know wth was coming. Friend died at khazra staff in NM (ice + arcane), A3 exploders in NM, belial in NM (wiz without crystal armor), phase beasts in hell (horde). Outside of the group I died to puking zombies at level 10 (before the patch limited them) trying to get a room full of them so I could do a massive AOE massacre. And my friend died to arcane waller in the sewers.
As for inferno, my friend's only deaths have been to playing at severe 1s lag and dying to a normal zombie that wasn't rendering. And a bunch of midgets that chain frozen + poison (barb with no way to escape without enough fury for WOTB).
My point being that while the statistics are neat, it's only really useful for bosses. The other kills are most likely from some kind of elite affix but we have no way of knowing. I would love for Blizz to record what actual spell killed people. It seems that frozen is the biggest killer of my friend group, coupled with arcane/molten or something.
