All their bodies are accounted for actually. I never noticed it either but the non lootable bodies on the ground account for them position wise and everything.
They would both be incredibly old. In fact, with how old Cain appeared to be 20 years ago in Diablo 2, I'm amazed he's still alive.

Cain looks different in D3, as if they've ran out of age for him, so they changed his appearance a little. Or perhaps that's the 3D rendering.
I think Leah could be Adria's child, and because she looks so much more *prettier* is because they are trying to appeal to different people, in my opinion.
Well, I think Adria just has more personality now. We obviously didn't have detailed in-game graphics for her in Diablo I, and I can't recall any official Blizzard art with her in it, so I don't think they really changed anything about her, just defined her. I like her more powerful jaw, in particular.
And, I'd say the same about Cain, except that now I think he's more hunched than before. I still think he should be dead logically based on the living standards of Sanctuary, but then again, he is a Horadrim mage, so he could have some form of special power or some such. I don't think it's ever really talked about.
I haven't read anywere that Adria was spared from diablo, in cains diary he says everyone was killed. Meaning her too wouldn't it? It never said anywere that she packed up and left before Diablo came back.
Then again she could be dead...I mean look the art work shows a young woman...not an old hag, which she would be round about now....Cain and the barb both aged.... The Barb looks in his 50's now, Cain...well he looks like shit :/
She should at least look 50 60, Her voice in D1 made her to sound very old as it was.

EDIT: Ohhhhh, he talks alot about Adria at the end. Perhaps I am mistaken.
