Sorry, I've not been around so this has probably already been discussed, but I've noticed a new work in the Art section of Diablo III, one depicting the Mistress of Pain, whom I assume supplants Duriel.
There seem to be two pictures of her; one more demonic in appearance than the other. I wonder if there might be some significance behind that...
This raises a few questions, such as; How does a Demon acquire the status of Prime or Lesser Evil? Are the Lessers just pretenders, forever existing in the shadow of the true Prime Evils? Are the Lesser Evils avatars of true natural forces? Or are the names -- Pain, Anguish, Hatred -- just names of realms of Hell, the lords of which name themselves after them?

Be it through hallowed grounds or lands of sorrow
All in the Forger's wake is left bereft and fallow
Is the residuum worth the cost of destruction and maiming;
Or is the shaping a culling and exercise in taming?
The road's goal is the dark Origin of Being
But be wary through what thickets it winds.
-Excerpt from the Litany of Residuum;
As Translated by He Who Brings Order
I think the significance between the two is that they where worked on by 2 different artists, one had the original Idea and the other decided to improve on it. or its the same artist but a year or so later decided to tweak the concept a bit.
There are no new lesser evils (Unless they add one and it is not included in the manual) Seven are the rulers of hell the three lesser evils Duriel the Lord of Pain Andarial the Maiden of Anguish, Belial the Lord of Lies, and Azmodan Lord of Sin.

Hell is a very chaotic plane. Unlike the High Heavens nothing is stagnant there. Who knows what weird mutation occurred in one of those demons or what bizarre creature might have seen the light of day (well not literaly) in one of those dark abysses.
I guess that's how the Prime Evils were created in the first place, what if Diablo was at the beginning a small red lizard lol.

"Seven is the number of the powers of Hell, and Seven is the number of the Great Evils."
Duriel, the Lord of Pain
Andariel, the Maiden of Anguish
Belial, the Lord of Lies
Azmodan, the Lord of Sin
These are the true names of the lesser of the Great Evils. For ages uncounted each have ruled over their own domains within the Burning Hells, Seeking absolute dominion over their infernal brethren. As the Lesser Four continuously vied for the control of those forces that dwelled within their realms, the Greater Three held absolute power over the whole of Hell. The Lesser Four used dark and evil measures in their quest for power, and herein begins the legend of the Exile.
Mephisto, the Lord of Hatred
Baal, the Lord of Destruction
Diablo, the Lord of Terror
These are the Prime Evils of Hell that wielded their power as a dark, sovereign triumvirate. The Three Brothers ruled over the Lesser Four by brutal force and malicious cunning. Being the eldest and strongest of the Evils, the Three Brothers were responsible for countless victories against the armies of the Light. Although they never held sway over the High Heavens for long, the Three were justly feared by enemies and subjects alike.
...."
And that will never change. New monsters can be made, but they will not change the order of things. Kabraxis is more powerful that quite a few of the above mentioned demons, but he is not a Lesser Evil, neither are Lilith or Lucion.
I will say it again, "Seven is the number of the powers of Hell, and Seven is the number of the Great Evils."

-Humankind cannot gain anything without first giving something in return. To obtain, something of equal value must be lost. That is alchemy's First Law of Equivalent Exchange. In those days, we really believed that to be the world's one, and only, truth.
At the risk of sounding like an ass, they are called Manuals, one would do well to read them. Or try the Arreat Summit.
Or read the moldy tomes you find in the games. More often than not they contain the writings of Horadrim Scholars, or the occasional rantings of a delusional, demented, sarcastic, egotistical, megalomaniac psychotic mad man.

who is sitting at his desk in the blizzard headquarters thinking of some more crazy $#!& to add to the game.
I'm sorry to say that I'm at odds with you here. While I am absolutely certain that the powers remain constant, if one were to be slain in a more "permanent" manner (wheter that is possible or not is debatable), I can definately see others taking up the mantle of previous evils. In this case, Duriels'.
One would first have to assume they can even truly die. Drognan says " I heard you were the one responsible for sending Andariel back to the Burning Hells..."
He does not say you killed her, nor does he make mention of the fact she is dead. If indeed she even is. He just says she is back in the Burning Hells. It's an odd line to be sure, as it can be taken in a multiple of ways.
