Just by playing, really. I wasn't able to play for days at a time since I work from 8-5 every weekday, so I missed out on the initial sell-Inferno-gear gold-making method.
I started about 3 weeks late to let the issues such as server stability even out......I spent tons on training crafters and still hit the 2mm mark without ever once selling anything on the GAH.
Nowadays my balance shifts between 5-15mm and usually never dips below 5mm for long.
My initial millions were made farming goblins on my WD. I gathered up some cheap MF gear and went at it. I started finding good things, like high level weapons and armor. However my first big break was when I found a radiant star emerald plan off azmo
You have to use GF or MF gear. If you don't use either one it's going to take a while to accumulate gold the way things are now.
In the beginning you could sell garbage gear for a decent amount of gold and repair costs were a joke. So people who played from day 1 had an easier time. Also, you used to get more gold from drops before I'm pretty sure. It's a lot different now.
If you enter Inferno today with no gold and obviously terrible gear (maybe you'd have some gold if you didn't buy a weapon for Hell) you're in kind of a bad position.
Basically you have to just farm 1 million gold somehow. Either by playing previous difficulties or maybe just not dying in Inferno.
Once you get like 1 million you should buy a weak MF set and farm A1. Eventually you'll get an item worth like ~5 million probably. Then you upgrade your MF set, etc.
You could use that 1 million gold to buy GF gear, but it's not worth it really. GF is bad compared to MF (although this wasn't the case early on) and it's insanely boring running things over and over for gold.
So basically you just play the game with no GF or MF gear until you have ~1 million gold. Then you use that gold to buy a cheap MF set that can complete A1 (not hard to do). And just farm A1 for a better MF set... and so on.
Of course people go other routes since they want to do things faster... which means using bots (not recommending this) or real money to get ahead.
I never had any mf/gf gear or really much focus on farming hard. I made my first million the same way I made all the rest of my gold, full act1 or 2 clears nightly (3-5 nights a week) and another 20 mins or so looking over the AH listing/buying. I make it a point not to spend much, penny saved and all that.
Some luck pushed me along the way from 1mil to 10mil (a nice quiver I think it was), but most of my consistent gold came from selling mid-range gear at really competitive prices, along with gems, crafting mats, tomes, and just about anything else someone would pay for. Most players appear to simply farm for ilvl 61-2+ rares and ignore everything else. They want the most rares for their time. I know I only have time for a single clear, so I just make the most of my NV stacks and everything that drops.
Its taken me a while to get to this point (buying MF and GF gear), I've got a MF set that gives me a base of 271 MF and I also got a GF set that gives me around 220 (only 7 items of the 12 have GF, so still improving this).. so I play act 1 first mission to Skeleton King, starting with GF gear, if I come across elite packs, I quickly change the GF gear manually for my top items to get max DPS, just before the last elite kill I use gear switcher to switch to MF set.. then continue farming with GF set.. last night I played for 3 hours, collected 200K gold, and 1 item I could sell on the AH.. so potencially I could make 1 million from last nights run depending on the AH item selling. Monsters typically drop around 300 gold, a lot of the time they drop high amounts.. I see 1800+ drops often, so its fairly quick to build up your gold. I died around 5 times within the 3 hours, so repairs wasnt too harsh.
I still havnt exceed 3 million.. but this is mainly due to building up enough gold to get an MF or GF item..

It's really quite simple. You either buy gold, farm gold or hope something drops that's worth big money. You can do OK by selling lower end gear but as more and more people get geared up, it's becoming difficult to make any real money from that. Another thing you can do is find good vendor rings or ammys but that can be hit or miss and you really need to know what's selling to make money that way but it's a great source of income if you find a killer ring/ammy.
I sold some high DPS 1-hand axe for 5 mil over the weekend so that's how I make the majority of my gold. That was in Act 1 with 5 stacks of nef and maybe 200% MF overall. I got a set piece that's probably worth between 75 - 100 million based on prices I've researched. That dropped from the Butcher. It's all luck but with MF gear you can make things more consistent. As they say, chance favors the prepared mind (or toon in this case).
Made all my gold in BEFORE 1.0.3 Made 5m since 1.0.3 almost 5-6 weeks ago. Guess what I'm doing now? Waiting for GW2. Not worth playing D3 anymore. 5m in 5-6 weeks is stupid luck.
I actually just farmed gold for a few hours right after hitting 60. With a 240% GF set I made about 500k per hour just facerolling though ActIII/Hell.
It's so mind-numbingly boring, though, that I hardly do it anymore. Damned if I know why, I've had the worst luck loot-farming for the past few weeks, I'd probably have made an easy 20 million more if I'd just gritted my teeth and ground out the cash. There's something annoyingly compelling about the possiblity of finding something valuable that make us put up with the reality of endless piles of +int crossbows and +200 dex affixes on 220DPS daggers.

I guess you burned out before the patch hit, it's still trivially easy to make gold if you are interested in doing so.
My first break was when I got another loot of blackthornes medal that has some trash stats (yes, it was the 2nd time I looted blackthornes medal), so I sold the 2nd one that has some trash stats for 6m and it was sold instantly..
Actually I even earn more these past weeks even though many people already quit. Most stuff I sell around 100k to even 3m... I net more or less 500k to 1m per day... Not bad than before.
Almost all of these loots are just from Act 1 with base 256 mf. Sometimes I do flip items too, but not that much.
As for RMAH, it's totally declining on my end, I used to sell stuff even trash items quickly, now even good items cannot be sold.

Of course you don't have to have it since NV is 75%.
But the differences between farming A1 with 75% MF and farming A1 with 275% MF (very easy to get) feels pretty extreme.
Getting no rares on bosses is really discouraging.
If you can farm A3 with only 75% MF from NV then by all means do that. I think it's still worse than A1, but probably only by a small margin. I'm just saying... farming A1 with 75% MF is not efficient.

Did you guys just farm Inferno until you found a godly piece and AH'd it or did you actually just farm out the gold? Or some other method!
