No one ever said a glass cannon build should also be resistant to damage (death). If your damage so vastly dwarfs your defense then be prepared to die under certain circumstances.
Reflect damage has never been a problem for me. Maybe because my highest character is only 45, so I don't really know about inferno, but maybe, if you do too much dps, you should slow down? That way you give yourself time to heal or whatnot. Or just get better survivability.

I think allowing purely glass cannon builds (that still require quite a bit of skill in terms of timing their defensive skills and positioning properly) was a conscient decision by the design team and might even be a form of homage to Diablo 2.
Sorcs, Amazons and Necromancers could be very item independent and go full glass cannon and do pretty well with just proper usage of skills.

I'll admit some mob types in act 3 and 4 still give me a hard time, but I as a DH can effectively farm Act 1-2 with minimal to no deaths. The repair costs feel right about where they should be; IE I'm droppin 25k to repair every 2nd or 3rd farm run, in which just the blues I've vendored has made me closer to 200k. I'm also purely glass cannon, able to survive 1 hit in act 1 and that's about it. It's just all about movement and timing. Gotta get that skill.
