Ive been through all kinds of solutions for the FPS drop during gameplay. None of them worked until I found the following solution. This is happening to many players as we can see. This issue only involves multiplayer, especially when 4 players encounter a lot of mobs.
Blizz has no solution but I give it a good try:
Head over to this location:
C:\Documents and Settings\username\Documents\Diablo III
Open up D3Prefs.txt in a text editor and modify the following highlighted options to match the image.
This SOLVED THE MAIN PROBLEM for ME! I won't go through all other possible tweaks - as there are too many (which dont work ;-)).
Before changing this I had FPs DROPs DOWN TO 15-20, now I DO NOT DROP BELOW 50FPS! When shit hits the fan with crazyness going on I might drop to 40ish. If nothing crazy is going I play at a constant 80-100 + - FPS during 4 player and casting.
My System:
Asus GTX 780 ti
i7 4790 @3.6GHz
16GB Ram
D3 on SSD
BenQ XL2420z Screen @120Hz
SETTINGS:
V-Sync OFF, other settings dont really matter I suppose. I did limit the FPS in fore- and background to 120/50. Rest on max quality. No NVidia tweaks. All on performance - as your PC settings dont really matter - the problem lies within the corrupted D3 settings/programings.
I did fix my stuttering a long time ago by dropping the audio channels to 16 from the ingame menu. Problem is some of the sound won't play anymore or they'll just get chopped off.
The stutters only happen in rakkis area and a few others due to how they handled sound (no idea what they did) but yeah.
Guess it depends if you rather have all the sound playing properly or way less stutters in those particular areas.
True. The problem is moslty baed on the sound channels. I do notice some drop in sound quality when setting it to 16. However I rather have no lags than the best sound. Hopefully this will get fixed with 2.1 by blizzard.

Another thing that helped me a lot on FPS is to lunch Diablo client directly and not with the luncher and after login , alt+tab bring the task manager up and kill the agent.exe as well.
+1000 to this! I tried the audio thing and it helped about 10 frames or so but was still sitting under 25 in combat. Im thinking its either the trilinear filtering or the hardware class. Whichever it is, i dont drop below 50 now! Yay!

Hi@all, Im glad that it worked for the most of you! Please Keep in mind, that this Tweak might not work for everybody.
The Problem that affects the most would be the Sound channels. Reason to this is, that the CPU only uses 1-3 hyperthreading, instead of using all CPUs for the Sound. Lowering the channels will kill some Sound Quality, but enhance over all gameplay.
(u can check this while u alt/tab during gameplay and u will see only see 1-3 high CPU performance, rest ist low)
Trilinear filtering and HardwareClass will lower a bit the graphics, yet enhance over all gameplay
@ZioNNN, if nothing help you increase FPS, try to completely reset you Nvidia Experience Settings to Default, using the tweaks above. And dont worry about ur CPU heating up. While only one hyperthread is being used to calculated the Sound channels, it will heat up like hell, res of the CPUs stay rather cool. It's more of a D3 Problem than your PC.

The Problem is not your CPU cooler, it's the fact that there is mostly only one CPU being used to calculate the Sound channels, therefore it will heat up like hell, the rest of the CPUs are rather cool. This is not your PCs Problem.
Hello, i have problem with diablo, my specs : i7 5820k, gtx 760 oc, ddr 4 2133, MSI X99s Sli plus.
It's realy annoying, without v-sync i have screen-tearing, with sync on, i have drops fps from 60 to 50/40 and it's feeling like 1ms freeze, lag. Without control fps in game options, i have ~300fps where i'm standig, when i'm move, fight, it's drops to 180/160 and these drops causes this freeze/lag .I've tried everything, change saund to 16-bit, v-sync off/on, change driver version, I have problem only with diablo, other games works fine. Sorry for my english. Please for help.

What might help: reset your Nvidia Settings to Default.
Try to Limit your foreground FPS to your Monitor max rate. (having more FPS than Monitors MHz, dont make sense anyway!)
Thank you for this, even tho its almost 3 years old it still works today. I just started laging horribly in season 9 I'm running a GTX 1080 with an AMD FX-8350 and 32 gigs of G.SKILL Sniper ram. Ive never had an issue with laggy games I'm on a 300 Mb connection, found this thread changed the config file as directed and lag is gone.... Thanks again

Right Click far left bottom of your screen on Windows Icon.
Select Search function.
Type in D3Prefs.txt
That should help you get your file if its misplaced somehow.

I found a mix that worked for me
Just Disable the Max Foreground FPS and the Max Background FPS.
you can also activate the V-Sync if you want it, but I am suggesting to not use the FPS limiter in Game they seems to mess with the FPS in so ways.
