My best guess would be the Triune, since in the Sin War everything they create has something to do with the number 3. We know that the cultists from the gameplay video are Triune cultist so it would make sense.
I'd like to think that members of the Triune all carry some sort of three pronged symbol and that whichever Prime Evil they serve, that particular prong is the longest.
It certainly seems plausible. But it's probably just coincidence.
Like said, if this is a sign of the Triune then why is it not seen on any of the cultist art?
Also, the Triune worship the three prime evils, why would Adria help the hero if she was a member of the cult? Unless everything to this point was just a plot by the Prime Evils to get the World Stone destroyed.

Maybe it has some occult meaning? Maybe a symbol for Diablo or the Triune, or one of the other Greater or Lesser evils.
