I found this interview by looking at the source code of the page for the beta announcement of D3. It's a short interview but it has some pretty cool stuff, I'm not sure if it was part of the press kit or not. I'm also not sure why it's not available from the side menu on the page, maybe it was meant to be released later?
Interesting tidbit about D3's difficulty modes. Kevin mentions "hell and inferno" as being the games harder difficulty modes, I'm curious if it goes Normal, Nightmare, Hell, Inferno?
This picture shows that Nightmare difficulty is indeed in the game. Four difficulty modes!?!
Blizz is not responding on the BNet forums, which makes me think that it really is a 4th difficulty and they didn't mean for this to slip out just yet.

This might just be enough to make me forget all about online-only. I'm ready for some ass-kicking end-game that requires incredible gear, level 60, and a fully polished and refined build with all Level 7 Runes. I want a difficulty that makes Completionists (like myself) cry.
The only other thing I can see this being is that they have different names for Hardcore difficulties, Inferno being the replacement for Hell. Only time will tell.
EDIT: I want Fallen that are comparable to Nightmare Diablo.

Difficulty names were the same for softcore and hardcore in D2, so i highly doubt they will have dif names in D3.

Notice that the sentence (Ultimately, it’s going to take the game's harder difficulty modes -- Hell and Inferno -- to challenge the limits of the best Diablo III players.) skips nightmare mode. There's no reference for what the two modes--Hell and Inferno--are harder than. It could be that they changed the name of Nightmare, we don't know.
The images we have seen might have been taken in an older build. Things could have been renamed.
Personally? Yes, I agree that it's probably another endgame challenge. But that does not mean that we can take this as fact simply because we want it to be true. Maybe there's a fudge cake in front of me because I want it to be there. After all, I'm hungry. Maybe because I'm hungry the cake exists.
Would be pretty intense to see that go down. Can't imagine the gear you will find there though.
It could simply be that the gear is spread out over this new difficulty (assuming it exists). So, there's a shallower slope to the hypothetical gear drop graph over four acts.

And how long ago did they create the press images for the press kit that was handed out to each site? The sites didn't take their own pictures. They were given pictures, as far as I understand how the press kit works. They could have been from a month ago.

I'm not saying you're wrong, and I'm not saying I don't want it to exist, but we're filling in blanks with what we want to be true. It hasn't been confirmed yet. We have no context.
