I really love people complaining about no enrage timer. If you have fought Malthael at all on any torment difficulty, you will realise just how difficult the fight actually is. I don't care how much toughness you have, if you fight him on Torment 6, he is going to hurt you and there are situations where you can and will die. To be able to do this for 3 hours on a hardcore character is truly epic. Congrats man!

But there was some weapons thrown and Malthael refused to take any damage for three hours. Tho, was some quite close calls, Second part of the first phase with adds and the skulls floating around is quite tough
Oh look, people getting upset that someone else did something they haven't done in a way that displeases them. "Oh, 50 million toughness? Must have been easy." "You only did him on T6, no rares/elites? Doesn't count." "He doesn't have an enrage timer? But all HC characters are supposed to stack all damage and no toughness and zerg boss down to meet enrage timers!" Unless you've done it yourself with what you are criticizing, you shouldn't be talking.
There is nothing wrong with the playing the game how you want to play it. Stacking 50 million toughness is a legit means of playing the game. There are no "rules" that say you need to win via blowing things up before they hit you. In fact, nearly every enemy in the game plays in the same manor. Players do not have enrage times on them and nearly every enemy has more toughness than damage output. Consider Malthael another player and you'll realize that Malthael too was stacking toughness (1.1 billion health on Torment 2, not sure on higher difficulties) over damage. TheFriendly15 also didn't "enrage" after 15 minutes and start doing 1000% more damage to Malthael on the basis that Malthael hadn't killed TheFriendly15 yet. Why should it work the other way around then?
Enrage timers work fine in games like World of Warcraft, where groups of players can "exploit" game mechanics in their groups (for instance, stacking healers against Thok to kill him in first phase, an unintended way of doing the fight by ignoring the rest of the fight's mechanics). For some other bosses, in other games, enrage timers don't make sense. Players should be able to decide how they want to win a fight. Don't get upset because you didn't do it first, because you didn't think of it first. If you're really that mad, just say "Congrats" and go do your own thing, or don't reply at all.
First off, a Big Congratulations to OP. Well earned kill, excellent job.
Enrage timers add nothing to a boss fight....can you imagine actually fighting one of these 'bosses' in real life, and if you didn't defeat it in a man-made unit of time it suddenly bursts into flame, and destroys you? Its much more interesting when its, I can do this, and the boss can do that, how do I overcome it? The winner, regardless of time, will be chosen.
The only enrage I'd ever want to see on a boss, is something like as health decreases they gain strength or abilities; but then again doing that doesn't really make it an enrage, just a part of who that boss is.
Not to take anything away from this dude but it is a bounty. Bounties are far easier than the actual campaign boss. World first campaign mathael still up for grabs.

Anybody complaining about his achievement = jealous dbags that have not played hardcore, play SC T1, and have the intelligence of a gnat.
Enrage is only a gear check, it's not cheese if you can stay alive and out battle the boss.
