i would take into account that the prime evils do not need soulstones, also there are nine gates(not to be confused with the Nine gates of the kingdom of shadows) of Hell, one for each of the Nine Hells, but otherwise your storylines are pretty good

Perhaps you could assign the 7 Evils we know of (Andariel, Duriel, Azmodan, Belial, Diablo, Mephisto, Baal) to guard 7 of the gates, and have 2 greater evils guarding the lower 2?
Possibly but each of the Hells has a lord already, you can search the net for all that info, the Prime Evils although the hail from the burning Hells the fourth Hell if i recall they are basically Lords of Lords, they are the, as their title suggests the Prime evils, no daemon is greater than they.

What about Tal Rasha? In diablo 2, act 2, you're going after Tal Rasha until you enter the tomb and find Tal Rasha has fleed and you are left fighting Duriel.
Everybody has mentioned Tyrael getting corrupted by the worldstone, but how about this.
Tyrael doesn't just become corrupted, but the tainted worldstone that contains the essence of baal turns Tyrael's body into a vessel of baal, thus becoming his new body. Tyrael-Baal tricks heaven into thinking he is still Good-Tyrael and secretly taints angels over a period of time, making an army of demonized-angels. Tyrael-Baal starts a war in heaven, or an Angel Civil War.
Meanwhile, Azmodan and Belial (as stated earlier) summon diablo back into the world to take charge over hell. Noticing that Baal is keeping heaven busy, Diablo makes war on Sanctuary, and that's where our new heroes are, and the story begins.
Maybe, I really like the idea of Tyrael actually becoming Baal, instead of just a minion.
Hmmmm. Very interesting idea. Perhaps when the game starts, all realms are under seige by the forces of Hell, Heaven being the least saturated, Sanctuary being moderatly saturated, and Hell being grossly saturated.
