I am well aware of the runes needed to make BotD. I am playing a lan-exclusive zealadin with my gf and would like to make a botd berserker axe for him.
I know how to upgrade all of the runes.
I also know how to upgrade weapon classes.
I have a couple quick, and hopefully easy, questions.
1) Does the axe have to be ethereal to make botd?
2) What is the route of weapon upgrade I can use to make a berserker axe via the cube?
3) What is the best way to get 6 sockets onto it?
If there are any other tips to getting this amazing weapon, please let me know.

1) No, but ethereal adds a hidden 50% damage bonus, which is why (along with BotD's indestructible modifier) it's preferred.
2) Ummm....don't think you can upgrade regular, non-magical items in the cube. Check Horadric Cube Recipes (Diablo II) for everything you can do.
3) Act 5 socketing quest. The quest always adds the maximum number of sockets to an item.
Good luck getting the runes you need to make it in SP though...
These combinations only work for Single Player, Open, or Ladder Characters.

I'm more looking for which weapon will turn into a berserker axes, and which one will turn into that. So I know which weapons to hold on to and upgrade :-p

Those are for unique items. You can upgrade the base item a unique item is made on, but you can't just upgrade regular white (or grey)-named items, which is what you need to make a runeword.

2) You are only able to upgrade rare and unique items via the cube recipe, so therefore u can't upgrade a naga to a berserker axe.
3) There are a couple of ways you can get a berserker axe with 6 sockets a.) find it with 6 sockets already in it B.) Have Larzuk socket it via the quest for max sockets, in this case 6. and c.) you can use the cube recipe, Ral Amn and a Perfect Amethyst, and try to get 6 sockets, however this will be random(1-6). Also, don't try and use the cube recipe if the item is superior, i.e. 15%ed or 10% increased durability, because it won't work.

You should be looking for an etheral berserker axe, preferably superior with 15% ED(if you're lucky it'll also have 6 sockets but GL with that). Then you'll take it to Larzuk and get 6 sockets.
Then even if you have all these perfect qualities you still have the insert the runes and cross your fingers for a high ED. 400% is best(with 15% extra from superior) and ~15% LL something.
I know I need the runes. I'm working on getting those.
I know when I make the runeword it's a random %
BIG NOTE HERE - per the horadric cube page, many extra things can be done in single player that can't be done online. This includes upgrading runes all the way to zod.
Also - Thanks for the information regarding the upgrading weapons. I was fairly certain I could take a white war axe and turn it to naga and then naga into berserker.
Any idea what difficulty and act we start to see berserker axes drop?
Nah, I don't think I've ever seen one drop outside of A5. I bet you could probably find them in the Chaos Sanctuary, but I've personally seen the most drop running through Halls of Vaught.

This is referring to UNIQUE weapons (Gold text). A gray or white text zerker is not unique, it is normal. You can not upgrade a normal zerker. You need to actually find one in the game. Chaos runs have worked well for me for finding zerkers. Still not impossible to find.
Also, not sure if you have done the math on upgrading runes to get a Zod, but it is a real lot. Nearly an impossible amount of high runes for SP.
