The drawings are supposed to be by Deckard's own hand; but they were created by a variety of artists, including Brom. So yes, some have taken artistic lisence with their work, and not all of the pictures match up when depicting the same things. It's all to be taken with a grain of salt; the only thing that's totally canon is the content of the games, bypassing the books and comics.
As for Lilith's depiction in the books, Deckard states that she has the ability to take on more than one form, so this isn't necessarily contradictory.

Imperius, I'd agree, becomes more tyrannical without Tyrael's balancing influence, leading him to take on the Aspect of Wrath.
Ithareal's lost his prescient ability where Sanctuary is concerned, and it's said he's gone mad. I'd guess Auriel either kills Ithareal; or perhaps Ithareal's loss simply affects Auriel to such a degree that she succumbs to Despair, therefor opening the way for her corruption. They are the closest of any of the Arch Angels, so what happens to one will effect the other.
And Malthael's gone missing some time ago, so he's the ultimate wild card. Will he come back to save the day, or become evil as well?
Asmodan is said to have generals that represent each of the Seven Deadly Sins. The Arch Angels are also representatives of "universal powers", and it seems each is being transformed, one by one. Could there be a connection?

Oh no doubt, the illustrations are gorgeous. As an artist, I can appreciate the time and effort that went into designing and crafting this book. Heavy stock paper that's been frayed at the edges; multiple artists' contributions, including the likes of Brom; cute little details, like smudges on a page opposite of a picture that's been "drawn" with a heavy hand and charcoal. It's a well-crafted book, there's no denying it.

Well Argile, I'm of the opinion that it's "Okay." I expected a lot more in the way of exposition, but it's honestly just a summary of everything that's happened in the Diablo world. It more or less catches the noobies up to speed. There's a couple tidbits in there, but it's a hook for new fish. If you want to know anything about Diablo, look it up in a wiki somewhere, you'll get more out of it in my opinion.

I heard early on that the male Barbarian in Diablo 3 is the same one as Diablo 2, aged twenty years. The Book of Cain implies that the Amazon, Necromancer, Sorceress and Barbarian all lived through the events of Hell, as per an illustration on the side. The only one missing was the Paladin, but that doesn't necessarily mean anything.
It's been my theory that the Templar Follower, Kormac, is the Paladin from D2. However, I'll have to re-watch the video in which he was initially introduced, since a bit of his backstory is contained in that.
The Necromancer, I've heard, appears in D3, probably providing some connection to one of the D3 expansions.
I don't know what happened to the others. At least one of them has apparently gone insane, but I don't think that would be the Necro.

See, I read it differently. Cain seemed to emphasize Mephisto's role as First among Equals, which effectively puts him in charge. Moreover, he's the most political of all the Lords, as well as most manipulative, so it's my opinion that, while he may have no official recognition, he more or less pulls the strings.
"Prime Evil" only applied to the Three Brothers, as they and their powers were so inextricably bound together. They fed on and off one another, a veritable machine in three components. This fueled their continued supremacy; it was only through the combined efforts of the other Evils that they were defeated. It's Brothers' unity that makes them so dangerous.

Kulle, a former member of the Horadrim, created the Black Soulstone in order to "feel alive" once again, after being left hollow from his ordeals in chasing down the Three. He sought to capture the essence of angels and devils.
It's curious that Kulle also speculated on the nature of the Abyss, where angels and demons are sent upon "death". This was noted towards the section on the Hellforge.
I think the Black Soulstone somehow taps into the Abyss, which will dredge up the Three as a unified Prime Evil.

I think it's established that it's Tyrael. There's a firm link between Tyrael and a mortal "stranger" in Book of Cain. I believe his actions at the Arreat Summiy have finally been uncovered by the rest of the Angiris council, and he's been banished, much in the same way the Prime Evils were banished long ago. As such, like the Evils, he now inhabits the body of a mortal, but strives to aid Sanctuary. I think he'll supplant Deckard Cain for a time as the players' adviser.
This probably establishes Imperius as an aggressor, who I think will be driven to malignant action by Malthael when he returns.
