Alright, well first off, in Diablo 1, i understand that Diablo was not in his true form nor was he at full power. he had been dormant for a long time and was just possessing Prince Albrecht for a current body.
Diablo 2, however, is more unsure.
A) he sheds his guise of the wanderer.
he's home in hell when we fight him.
C) he looks different, or at least his head does, in the cutscenes than he does in actual gameplay.

as for Mephisto, i think it's safe to say that he was not in his true form nor at full power in Diablo 2. rather, having somewhat transformed the body of his host.
Baal...not sure what the deal with him would be in LoD. with the time and freedom he had, i would assume he may have reached his own true form or full power, but maybe not.
so we know that Diablo survives. we don't know if his brothers have, though.
hypothetically, let's say they have, just to give us more bosses. :cool:

personally i find it a little ridiculous that human warriors could defeat their true selves, regardless of how the nephalem are supposed to be potentially super powerful. (Uldyssian was way too ridiculously powerful for a human.)
I really hope we see them in true form/ power in D3.
Mephisto in particular, he was my favorite boss back in D2.
He'll look even more badass in D3, since he's the fucking embodiment of hatred, I really can't wait to see how they handle him.
As for Baal, I'm pretty sure he was in his true form, if not though, we've got another new boss ^^.

Who knows of they have one true form though, they could be fluid and ever changing, evolving to best suit the challenges that they are faced with so that they can remain in power.
Or, maybe each time they are killed they reincarnate in a new form.

I'm pretty sure that it says somewhere that they are strongest when they are together, so I'm guessing that if their plans had succeeded, they would have all assumed their most powerful states.

Im not sure if this was ever mentioned, but I think that Diablo's power was still limited by the soul stone in some way. At least that is my personal opinion. In both instances we have Diablo taking control of another human body. First the prince and then the hero from D1. I don't think we have seen Diablo in his true form yet.

Well, my memory is a bit fuzzy, but I believe that that is what the Uber Tristram Pandemonium Event was supposed to be- the Three at their strongest together. Obviously, they couldn't make new sprites or whatever for them for the occasion for a visual change because they don't have those same artists or anything, anymore. But I'm pretty sure that's what it was supposed to be.
The PE's are supposed to be quests of a sort with a tiny bit of lore behind them, but most people don't really consider it cannon. I think when it was released (the patch for it) Blizzard put that somewhere- maybe on the Arreat Summit.

This is how I look at it. If I'm wrong, at least I was making a somewhat logical assumption. If I'm wrong I doubt there will be a very good explanation for the 3 mouths, we'll just have a Diablo that decided he would look scarier if he had a few more mouths to yell at you out of.

Yeah, I don't think Baal, Mephisto or Diablo decided to merge with each other in Diablo 3. To me that wouldn't seem very "diabloish" as it's more the kind of thing you see in some animes. I like the terror explanation however that Diablo has simply taken on this form to look scarier as that seems consistent with his personality and powers. It might also be his true form since he isn't inhabiting a mortal body anymore so I imagine he'd look more badass than he did in the past now that he's no longer limited by matters of the flesh.
One theory I haven't quite seen proposed yet is that the demon shown (Mr. 3-Head) is a very, very powerful demon that was summoned by the 3 Prime Evils -- not a combination of themselves specifically, but a kind of summoned conglomeration they've created as a champion of sorts.
I don't really think that's the case -- I'm not exactly sure what's up with the 3 heads, but it looks almost exactly like the rendering of Diablo shown in the new 'Overthrown' statue, so it would seem to be Diablo and Diablo alone, though in perhaps some kind of new / truer form; not the 'true' form, because I don't think 'we' (humans, the hero) can actually see or perceive demons in their true form, just as we can't see angels in their true form either.
I think we're all forgetting about Belial, the father of the three. When I first saw the cinematic that's who I thought of as the new enemy, and after all, he's the only lesser evil that hasn't made an appearance, as every other one did in diablo 2

i've never heard of Belial being of any relation to the Three.

i believe it was said that after the exile of the Three, Belial and Azmodan then turned on each other for control of hell and in a civil war, they destroyed eachother, although i could be mistaken.
so those two may not even be around anymore.
but as we know, Diablo continues to come back, so other great demons probably can, too.
They weren't destroyed. The lore states, basically, that in the absence of the 3 Prime Evils Belial and Azmodan basically command 2 sides of Hell in a civil war that resides (I believe) more or less in a continued stalemate.
They're around, and I keep seeing it stated here and there that at some point Blizzard said we'd be seeing one or both of them in D3 (at least one or the other), and it rings a bell in my head, but I'm struggling to find the source. I'll keep looking.
My personal belief is that the Triune Headpriests are tied into the 3 (a part of their soul or some such thing) and that it is they, along with many of their most powerful mages, manage to summon Diablo, Baal, and Mephisto back, but they couldn't summon each of the three seperately, and could only merge the three souls into one to create this Super Evil!

Hopefully the demon that was shown is just Diablo in his true form though, I'd really prefer it if the prime evil were kept separate.
