It should be worth noting, now this isn't related to difficulty or anything, but that there are random encounter and quest throughout the game. Basically you can walk down the same road and see a cottage one time with nothing going on, next time no cottage but a maiden being attacked by demon crazed wolves who then ask you to help her save her husband from being sacrificed in a nearby cave. Another attempt may just show a very simple cave entrance after you go in you may see a guy just run in fear and after you go and find him you realize he is possessed and losing the battle finally attacking you.
Stuff like that can happen at each stage of the game, so there is already replay-ability in that. Give Blizz time to patch in new content and such and who knows what we will see.
I think inferno adds a good place for end game farming, but just that...there stills need to be a point for the farming..a reason to make you feel that getting stronger makes sense even once you can beat all monsters easily...

I think blizzard will ultimately get more in depth with pvp, once people have actually dug their nails into it and get bored of grinding gear without purpose.
Adding some PVP content would be good for everyone.

But you don't feel like you want to do something with that loot? I don't know. Maybe like when you have the best loot in the game, only then you can kill [insert whatever you want here] and feel you have "beaten the game"?
For me, playing Diablo is like playing the Disgaea series. It's all about becoming more powerful and gaining the satisfaction from that. In Disgaea (3, for example), it only takes about 80 hours to beat the game. Then the game really starts and I spend hundreds of hours of grinding for levels and loot. By the 320 hour mark I already bested the most difficult enemy in the game (and this is a singleplayer game, mind you). But... people like me are addicted to getting stronger ever so. Each time I can get my damage output up by another 10% is a happy day.
One thing i would like to see that would really test your character in end-game would be the "Endless Dungeon" with added "Phase" or "Wave" mode. Well, i wrote a lot but then i figured i could just say "sort of like Killing Floor" (the zombie PC game). You are attacked by a wave of monsters. You finish them and then after a few seconds you are attacked by "the 2nd wave" and then the "3rd wave" and so on and so on. Each one is more difficult than the next while giving better loot each time. (Possibly always cirka the same ammount of monsters but each wave gives them +1 or +2 levels)
This way you could really test your character and all those stats you horded through items would really mean something. This could even be included in the Banner, saying that you reached Wave 15. But then you see a guy who reached Wave 20 and you think "Fuck, i have to farm more and beat that guy!"

Sounds like the horde mode kind of idea that a lot of people seem interested in, I for one am one of those as well so sounds like a good idea
