Something I really enjoyed about Diablo II was the titles attained after completing each difficulty; Seeing that little "Conqueror" or "Patriarch" in your name was really cool!
It's a mechanic that, not only would I like it to come back in Diablo III; But for Blizzard to expand upon it.
I remember back in my days of playing FFXI, you would be awarded a title for the last quest you completed, like dragon-slayer; Only downside was if you completed a small insignificant quest: your previous title would be no more, thus bunny-collector overrides dragon-slayer.
An easy fix would be the one found in WoW, where you can choose your title from a scrollbar
So you could either be "Kragdor the den cleanser" (act 1 quest 1) or "Kragdor the Hellslayer" (awarded upon killiing diablo)
Judging by how the boss enemies already have unique titles and WoW's new acheivement and title system is coming along, I think we might just see some more interesting titles!

I'd like: Anderonine, Guardian of Sanctuary (Maybe for beating D3 Hell Hardcore?)
I think it makes more sense to have your title represent the most challenging feat you have accomplished. This is already how it was done in D2 because if you beat the game on hell that replaces your title for beating the game on nightmare. This way it also serves to let other players know how far into the game your character has progressed. There's no point in having a title for beating a boss in Act1 normal if you've beaten hell, no reason to give the player this option...it would require more user-interface menus which would be a waste of time to implement for the dev team since it serves no purpose. If they have time to add an extra UI, there are much more useful options -- such as making a dueling-ante window, or other trade-centric UI's for coordinating trades.
I suppose it could be expanded upon by having a separate title for each act of each difficulty level, but this would require a lot of titles...like 15 different titles, it might get confusing. I wouldn't mind it, but I think the existing system in D2 was fine..since it doesn't take that long to beat the game on any difficulty level anyway, so you will just be zipping through titles as it is.
Id only assume this(in some form) would be carried into D3. Not sure if theyll expand on it though.

I think you should be able to pick between any title you've obtained simply because it gets boring to have a million "Patriarchs" and "Guardians" around.
the fact that there will be achievements might give the game a chance to award more titles. I'd like how it happens in Warhammer Online. Kill 200 orcs, you gain the title of Orcslayer. I wouldnt mind "FingolfinGR Demonslayer".
Blizzard might have learned from WoW on this too. After completing some certain quest chains, you could gain some ranks. If there is ranked PvP there are also titles there. My main in WoW achieved the title "Champion of the Naaru" when it was first released. Felt quite awesome too, i must admit
well, Naaru are beings of power, very much like the one wielded by paladins. The quest chain to gain that title needed for people to complete several group quests at first, several heroic dungeons (that were quite hard at the time) and finally complete a few raid boss encounters aswell. Given the effort needed and how hard it was at first, the few of us that managed stood out in the crowd with these titles. That alone was reward enough for the effort. But there were also loots while we were there too
Titles based on accomplishments and current streaks would be cool.
Lets say I have leveled to say 50+ for example and have not died a single time. Kragdor the Immortal. And... if a person who had this title died, Kragdor the Slain.
Again, storyline accomplishments like Hellslayer seems good, maybe others such as a certain amount of undead killed granting Undead Hunter etc.
