in my opinion and if the game is going to be dedicated to the lore of the older d2 stories. diablo and his bros are just waiting to take their shot at taking sanctuary for themselves because the balance demands that they exist or sancuary will become stagnant.Note: i dont want them to eventually win nor do i want them gone or this wouldnt be a true sequel without diablo. hell id settle for a diablo stuck in limbo like sauron in lotr
It's probably just Diablo. I severely doubt that the Three are merged in one; the secondary heads are far too small and bear no resemblance to either Baal or Mephisto.

In the previous two games, Diablo and his two brothers, Mephisto and Baal, were the three Prime Evils of a world called Sanctuary. Where we start off in Diablo III, it’s twenty years later (after Diablo II: Lord of Destruction) and the brothers are gone, they’ve been vanquished. Essentially everyone was kind of geared for Hell to actually invade. At the end of Lord of Destruction, the Worldstone was destroyed, it left a giant crater and everyone was expecting the hordes to come pouring out of it and nothing happened. So, Deckard Cain is one of our main characters from the first two games,he’s spent the last two decades trying to find out where the last two Evils are, what they’re doing and why the invasion didn’t happen. A lot of the rest of the story focuses on Tyrael, the Angel of Justice, and what’s happened to him since that event as he was actually the angel who destroyed the world.

Yes, that's been suggested countless times. The possibility that Belial would appear as a three-headed demon was even held in discussion for a long time before the announcement of Diablo III/release of the cinematic trailer.
They are the rulers of hell, even defeated the Prime Evils.
They are super badass and I hope to fight them
Yeah I kind of forgot that part, but I don't think just the two of them single handedly defeated the three, I think it was more of they lead an uprising against them.
If two lesser evils defeated three prime evils, then the hierarchy doesn't make much sense.
Yes I have to argree we WILL see the 3 prime evils in this game and most likely a whole load of new lesser evils,Mephisto may be a bit weaker because he keeps getting older and Baal most likely will come out of worldstone crater somehow,Diablo himself seemed to be in the Cinematic trailer right befor the girl said "I don't think it's safe here" and then the D3 logo came up,he sort of screamed or yelled or something. Tyreal too was foound in the citematic trailer throwing his sword at the worldstone just like in D2 "Destruction's end".

Diablo and company were banished to the nethermost reaches of the Dark Abyss, from which their essence originally spawned. They are not dead, but vanquished. Think of Sauron from Lord of the Rings. With the Ring's destruction Sauron lost the greater part of his very power and essence, and was never again able to manifest himself, corporeally. The same may be said for Diablo. They may return, but not for a while. They may not even be "conscious" of their plight, precluding them from taking any direct action, anyways.

If Belial and Azmodan are part of D3 I'm not sure the prime evils will be.
They don't seem to get along, and I can't imagine them all agreeing to forgive each other and work together.
How can you not see them being main bosses?
