I just died and can't rez, but I don't use 'sheild bash.' I do however have Akarat's Champion and I'm pretty sure it was active when I went down; the rebirth proc should have happened but only half worked. It behaved a lot like what was described above with the Jailer/Shield Bash proc. I appeared as the dead cross that always marks the spot when you die. Some abilities would work and some wouldn't, I couldn't move, eventually my life ran out a second time, and I got no opportunities for a rez. I had to leave game to correct it.
