All of the above. They are all well within the bounds of what I consider acceptable for microtransactions. And it's not like the voice actors/art team is heavily integrated in the mechanics-patches we'll probably get between now and next x-pack. This gives them something to do, but only the customers who are interested in what is on offer needs to pay.
I've seen a few people declaring this, and personally I don't get it... I mean the ability to do this is 99% luck, although insanely unlikely you could go from normal to t6 in 1 hour. Yesterday somebody in our clan gambled his entire class set within the space of a couple hours it's all just rng.

Does "searching high and low" include the Battlefields of Eternity? There are a ton of armor racks there. Just in case nobody has a spare/offers to give it to you =)
Haven't really sat down to analyze, but this looks better than the current system, where you need two socketed weapons (if class allows) or feel useless.
Because many people on Hardcore would abuse this by hitting Alt-F4 the instant a situation becomes hairy. You can see this happening in other ARPGs, like Path of Exile.
