If you can't see a character it is because he is in another folder and PlugY isn't looking there. That is the only reason a character won't show up, because D2 doesn't check the patch version or game version on characters before it shows them on the character select screen. You could have a character from Median XL in that folder and he'd also show up on the character select screen so long as he was in the proper folder.
I have no idea what your folder structure looks like, but so long as you haven't mistakenly overwritten him yourself you need to move him to the proper folder. Easy way to do this is to create a new PlugY character called "plugyfolder" (or something), see where that file is saved on your hard drive, and move your original lvl 54 character there.

You can't save the file? Do you have the folder set to read only?
In any case you don't necessarily have to change the save path, you just have to make sure the files you want to load are in that folder. Usually that should be a folder called save in the PlugY folder.

Problem is my character is now reset to level 1 with no items.
That seems highly unlikely, what's more likely is that PlugY has somehow started to look inside the wrong save folder and has found another character you've made with the same name. Make sure the PlugY.ini file points to the same location where your original character is saved.
