Goodguy was banned so long ago I can't even remember I'm pretty sure I was on the staff at the time too. It was probably flaming or trolling though I'm sure. That's 90% of our bans. I do remember he had multiple infractions and warnings beforehand though.

Goodguy accumulated something like fifteen or twenty infractions for serious trolling and flaming offenses. Then he created an account to circumvent his temporary ban that should have been permanent by the rules standard, and so he was banned permanently.
We aren't nearly as strict anymore (see our new board rules--now six thousand lines shorter!), but there are still some things you just shouldn't do as a decent human being
We don't use infractions anymore. They were only with vBulletin. Now we have IPBoard. They were basically just warnings. When you accumulated so many, the system would auto-ban you for incrementing amounts of time, the last of which was permanent around ten or something. They helped moderators and administrators keep track of user histories so long-term attitudes and offenses could be dealt with accordingly.
But like I said, not nearly so strict or organized anymore
(Although, I believe we have "warning levels" now, which are almost the same thing.)
