If there is a fourth difficulty it's probably insanely hard.Something like creatures in Hell range from 45-59 and then everything in Inferno will be 60.
- could be one of the endgame, where we grind for ubah loots, no more junks drops here so loots are a lot less but when loots do drop they're usually high quality, adds excitement to keep moving on to the next wave of encounter.
- since AI is much smarter in this game, monsters in this difficulty are far more aggressive than before not just added health, speed or resistance (monsters actually strafes on your attacks more often). normal monster now gains a few behavior/traits/skills that lieutenants/elites/unique/bosses have.
- or this could simply just the name after you picked hardcore difficulty.

I love all loot fest kind of game! I will be playing all of them for the next few years. Loot fest games I'm looking forward to: LotR: War in the North,Torchlight 2,Borderlands 2 and of course Diablo 3.
Ok, so Inferno is confirmed to be a difficulty beyond hell. I like that, but if this is Blizzards idea of endgame I can't help but feel a little disappointed. I expected something more elaborate than "all acts runs" as opposed to Baal runs. Though as a (semi)completionist I'll play until I've done most, if not all, achievements (which would logically result in a fair bit of play in inferno).
We can't expect them to add more content. The game has a story which has to be upheld, and any extension of that can be done via expansions.

They tried something similar to that and they said it felt bad, but that would be more like an extra game mode anyway.

I think you're underestimating the impact of additional abilities, new behaviors, and the randomized sections of the game.
Based on the comparision of some standard-vs-heroic versions of raid bosses from wow, the fights in Inferno could play out VERY differently from n-nm-h versions. Will some fights just be bigger/faster/longer? Sure. But others will have new mechanics and ways to approach them.
Imagine the "get out of the cave in 3 minutes" 'random' event where every single monster can kill you if it gets too close. THAT WILL BE AMAZING.
I mean, yes. There will be a resemblence to n-nm-h encounters, but there will be NEW and CHALLENGE. And that is a fantastic combination.
I love the idea, This is a mode where the monsters are a higher level than you. Plus they have new skills and are harder to kill. IDK what more I could ask for in a PvE challenge.
