Instead of thinking ahead when they end of the game has happened why not travel back.
Tyrel has destroyed the world stone, the soul stones are destroyed, where is there to go from there? There are a few options, but a very reasonable one, go back to where it all began?
See in the book for diable one, there where originally 7 evils, vonsisting of 4 lower evils, and 3 higher evils. Tyrel, izual, andariel, and hadriel, where the lesser evils.
The basic story line would be based around the time of the Horodrim, and Decard Cain. It would show the making of cube, and your goal to imprison the the 3 greater evils, and so forth. Learn how it began rather than what it becomes.

The perfect continuation from this would be to finally establish Sanctuary as a third power, equal to Heaven and Hell. It all depends on what happens in the third book of the Sin War trilogy though, but that's a continuation I find likely.
Inarius could also be thrown back into the melting pot, and to see what he wants Sanctuary to turn into would be intriguing to say the least, and whether or not Heaven will make take more active stand.
As for going back to where it began, that has already been suggested a thousand times already.

Okeey. First of all, Tyrael, Izual and Hadriel are all angels, not lesser evils. Two separate entities. The four known lesser evils are Andariel, Duriel, Belial and Azmodan.

Actually, from what The Sin War trilogy has told us so far, the Worldstone does nothing but prevent humans from using magic. Initially it hid Sanctuary from both Heaven and Hell, but once both sides discovered it, that hiding force doesn't work anymore. So I really do hope the third book explains how it managed to lock demons out.

I don't think they're changing, we just haven't seen all of it yet.
By the end of Scales of the Serpent, Uldyssian changed the Worldstone (something about it originally being made up of 5-pointed crystals; he changed it to 6-pointed) which according to Inarius should be impossible for anyone but him. That may have been what up until recently prevented demons from entering.
