It is unclear, but as it is increadibly blue, almost woad-like (Braveheart anybody?), you would not be wrong in thinking it is war paint, probably a plant extract.
How ever the character in my sig is a barbarian and his is not paint, it's a tattoo.

I always figured they were tattoos when I played D2. It's more hardcore that way. I always thought of the Barbarian as a guy that would willingly have needles jammed into his head repeatedly just to look that much more like a badass.

Traditional, or tribal tattooes are not done with a needle. A small chisel like instrument is use to cut a small groove in the skin, and a ink like substance is poured in to fill the gap.
