Ex:
4x Spaulders of Zakara from Kadala yesterday, all within 5 minutes
2x Natalya's helm within 3 clicks from Kadala
2x Natalya's boots within 2 rare pack kills (t2)
5x Ring of the Zodiac within 6 Rift Guardian kills (t2-t6)
Keep in mind, these are consecutive without seeing another legendary drop between them.
I'm not claiming this as proof, rather as data to observe and come to your own conclusion about pure RNG

I only have + fire damage on my 2h weapon, and can get it on my bracers if needed for a total of +36% Damage to Fire Skills. I currently run a lightning hammer build (http://us.battle.net/d3/en/profile/Moltres-1908/hero/44882667) with only +28% Damage to Lightning/Blessed Hammer.
I can easily run T1 rifts, and am a bit slower on T2 right now. Do you think with my current gear (if I re-roll bracers to fire damage) that this build would work? I want to get an Andy's Visage and a new neck (for fire damage) if RNG will let me.

I can walk up and melt an elite pack in a second or two in normal. 20 packs x 2 seconds = 40 seconds of battle time + 5 minutes wandering = 5:40
In torment, if we're assuming that you're taking 60 seconds to kill each pack. That's 20 minutes + 5 minutes wandering = 25 minutes
Math iz hard.

Here's a common scenario that happens and players mistake it as a bug:
You have 4 stacks of NV.
You kill an elite mob, and now have 5 stacks of NV.
No rare items dropped.
This is working as intended. Like the post above, if you are 100% sure that you had 5 NV stacks PRIOR to killing the elite pack, and no rare dropped, submit it to Blizzard. I cannot vouch for this bug, nor can anyone I know.
