To be honest, a lot of the posts here are very well written, all be it a bit over critical.. Farming Over 30 effectively on any class typically requires a "break through" drop.. one item that really pushes you past a break point, allowing for a play style that was otherwise unattainable. I have gotten my monk to just under GR40, my DH can complete 40's in just over the time allotted, and both literally require certain drops/stats in order to move forward.. all of which become harder and harder to find.
Your gear in no way "sucks"... its what you have found at this point in your gameplay/time commitment. If you cannot push past the RNG by throwing a ton of time at the game, you may literally never find the gear you need to progress further. There is a correlation between pg levels and player gear, time spent closes the RNG gap considerably...
All of this being said, follow the advice given above but make sure that you realize that swapping out items is typically a "more than a single item" thing.. one item at GR lvl 30 wont generally push you to level 40.. the entire build starts to matter at GR 30+ (gear included).. you are pushing beyond T6, and the difficulty curve is exponential..
