This is what I've come up with so far, but I'm struggling with how to display just about everything.
I figure the best course of action is to have all four levels of the dungeons (cathedral, catacombs, caves and hell) appear on the same page, instead of having four awkwardly named pages for the cathedral, then four more for the caves etc.
The starting section will talk about the level in general, and then each subdivision of the page will deal with the specifics of each level: what quests appear there, what monsters are here and anything else specific to that level.
The monsters aren't turning out so well though. The visual representation works quite well in D2 when there are only a handful for each location, but with 20+ monster (not including uniques) it just gets overwhelming. Should I just skip having visuals of the monsters, or solve it some other way?
PlugY for Diablo II allows you to reset skills and stats, transfer items between characters in singleplayer, obtain all ladder runewords and do all Uberquests while offline. It is the only way to do all of the above. Please use it.

That's just a fancy way of linking between different pages, which brings us back to the problem of awkward article names. Seeing as it won't be used anywhere else on the wiki either, it's going to be very counter-intuitive to use.

Maybe you could just link to a monster's family page and then list the individual types that appear on the level rather than having a separate link to each type. They all go to the same page anyway.
Or what about doing tables with collapse/expand? That way there aren't actually 100 monsters or whatever there, it's only the sections they choose to "expand"?
Only add the new monsters per level instead of including old ones.

That could work. If I use the "newest" monster to appear as the displayed image, it would still show progression but also include enough information about past monsters.
Now that I think about it, it might actually be more intuitive than simply listing all versions, which just blotches the whole page.

That is how I've solved it for the collective monster pages in d2 and d1 yeah, but I think the two pages are different in intent. The monster pages are naturally created to direct people somehwhere else, while the level page is there so people can get information about that page specifically.
Then again, the planned but never implemented affix lists for items in D2 also used this structure because of size limitations. I guess my main issue is that it prevents the viewer from getting an immediate overview of a level. But if Kickin's suggestion works out poorly this is probably the next best thing.

That's not going to work, since it needs to be clear when a monster disappears as well. For example, skeletons don't appear on lvl 3 or 4, which needs to be apparent.

I think it worked out pretty well (aside from the missing fiend images). It gives you an overview of what monster types there are while also telling you how many of each kind.

Only thing is I'd make sure to add links to the images too since that's always an issue when it's not done.
