Like another poster here, i also noticed the correlation between the two mouths on his shoulders and the two circles on Tyrael's armor.
It could just be Blizzard increasing the "badassery" of Diablo, or it could be directly related to the host of Diablo's new form: Tyrael.
I just hope all you guys aren't disappointed if they decide to go in that direction I've already seen some negative posts regarding the theory of Tyrael becoming Diablo.

Pewrsonally I think I would disappointed if this was not the case. It's the most badass combination and certainly a good way to end the story.
Maybe they are mad because they don't know which mouth to hit first.
I never said I would be mad if this was Diablo by himself, I'm just saying that the evidence that this is a merged form of the 3 Prime Evils is stacking up higher and higher. If this is simply Diablo's new look it would be fine with me, I just don't think that's the case.
Nice drawing, I for one hope that its not Diablo. I'd like to see new main antagonists besides Diablo/Mephisto/Baal. Nothing wrong with the old ones, I'd just prefer to have something new. I'm still leaning towards the demon in the movie being Belial.
I still don't agree sorry. I think Dauroth is right. Let me show you what it looks like to me:
See. It looks like each shoulder head only has one horn. The tentacles are coming from the back somewhere. You can see the movements of the base of the tentacles in the video. It looks like the tentacles aren't very long and are rooted locally - not all the way from the other side of its body like you are suggesting.
I'm not saying that the shoulder heads don't represent Baal and Mephisto, but I am saying that there is no physical difference between them.
That looks like the closest depiction so far, I believe both shoulder heads are identical, and that it has to do with 3 dying and 1 being brought to life. Mega-prime-evil? Seems kinda hokey and a gimmick, but I'll concede that that much evil being banished in a relatively short time could coalesce in the Abyss into some sort of single being with 3 consciousnesses. Schizophrenic prime evil if you will. It is then very possible that Diablo would win over Baal and Mephisto and take single control over the body. The body would then be mostly (80%) Diablo with touches of Baal and Mephisto (10% each, tentacles and horns, respectively). They could each have their own representative head, but they would no longer be sentient or really aware. They'd be trapped. This is the most logical way Blizzard could spin the megazord Diablo issue. This single entity would then go beat up on Belial and Azmodan, thus why they didn't invade for 20 years.
The candles are pretty compelling:
"It is said that in the end of all things..." Lights 3 candles next to each other
"You'd find a new beginning..." Lights 1 larger candle on top of a skull.
3 end 1 begin. *nudge nudge wink* If you don't think Blizzard would hint that, you need to catch up on their type of humor.
Now that that's out of the way, the side heads, if identical, each have at least 2 horns. One front thick horn sprouts from the top and middle of the forehead and curves back, one thinner horn would be further back and curve forward. This is all that is visible.
Evidence:
It is very possible that there are 3 horns per head, one in the middle curving back and one on each side of the hood in the back, curving inward like so:

The horn furthest from view would then be too low to be seen at any point.

The only problem is that the Prime Evil's spirits have apparently been sent back to the black abyss, and so are supposedly no longer in their soulstones? I honestly have no idea if this could work, they haven't explained it to us properly, so who knows what it will turn out to be! Its just exciting thinking about the possibilities! But you're right, Inarius would be a great option to use as a host body for something, assuming he's not all weak and weary from being tortured, lol! Your exclamation marks are addictive!
Dauroth's recent idea is actually really cool. As Atrumentis said, it's hard to say exactly what's going to happen since we don't know how the abyss will play into this... or even what the abyss is... But I like the idea of Inarius being used as the host body. I could very well work this into my fan fiction! Great thoughts here Dauroth.

Now listen to this:
What if someone collected the shards of each of the Soulstones, and then shoved them into one body! That could cause the Prime Evils' essence combined into one being!
Now, who would be the host? Could it be a human again? If it is, maybe that's the faith of Abd al-Hazir! We know the cultists kidnapped him, and we saw on some of the artwork that the cultists are actually from the Triune cult, who are devoted to the worship of the Prime Evils! Otherwise, it could be a random host!
My other theory of the host would be this: The human body is too weak to withstand the corruption, therefore, a stronger being is needed! An angel, perhaps? No, I'm not thinking of Tyrael, I'm thinking of Inarius! He's perfect! Chained for an eternity in the deepest recesses of Hell, this could be a final punishment for him! And, he's the easiest angel they could get to, and to do that to him!
Of course, there's a question how could one bring those shards to Hell! I have two theories!
The first one is about the Infernal Gate in Mephisto's Durance! What happened to it! Did it stayed open! If it's closed, who closed it? If anyone, the Triune cultists should be the ones to know of its existence!
The other theory evolves around Azmodan and Belial, or possibly some other demons we were not yet introduced to! They could be the ones to do this, although I doubt for Azmodan and Belial since they were the ones that left unpunished for the Dark Exile! They would probably do anything to contain the Prime Evils out from Hell!

What if the cultists are plunging the smaller shards into themselves? So before we face one of the prime evils, we're basically facing off against different tiers of that prime evil manifested in the cultists. Each one more difficult than the last, until we face a prime evil itself.

Im sorry but ill take this thread a little off topic... Did you notice how alot of the concept art is showing chained demons? Like the spider queen thing, and others... I noticed that when u said "inarius, chained for eternity"... I think it could be either some kind of a hint about the plot, or part of the story...Maybe its just that blizz artists like to add chains to their drawings...?
My opinion is that the three headed demon is either Belial or some mini boss. It sounds like something blizzard will do just throw in a small demon in a teasre cinimatic and then in game find out its like the first act boss. lol idk just my thoughts

It's just too far away from the older diablo models.
But hey, diablo could have been reincarnated all buddah-style into the body of a new demon perhaps :/

As I said... the design just seems too far away from the diablo from past games, while it is possible that this guy is him, I highly doubt it is.
I love the look of him though, he looks pretty badass.

I didn't notice a hole for the soulstone either, dunno if that proves diablo or not.
Although there is like a slit kinda thing on his forehead, could be a soulstone hole.
