Know that I look at it you could easily screenshot it, crop it, and add that text book in word. Until the link is provided i'm going with fake.
Edit* I found it on reddit the guy that posted it had been a user for an hour i'm sticking with fake.
So in "The Book of Cain" "Burning Hells: The Realms of Evil" section and the subsequent sections on the 7 evils of hell, three prime and 4 lesser, there is easily enough for an expansion. So if the book is true Mephisto, Baal and Diablo make up Tathamet's head (the book does not confirm this but speculates this to be true). Each of the 7 evils has its own realm in Hell. They are constantly moving and boarders changing. It is believed each Realm is at war with one another (books theory). In the following sections it gives a brief description of each of the seven evils. In each is a description of the realm that evil possesses. One act alone could be hell and all its realms (Azmodan's realm we played through). Really each Realm could be an act if done properly. We could easily start off in Aranoch and make our way through various places Cain visited (find each realm of evil in a different part of Sanctuary easily 5+ acts in DIII [already cleared Azmodan's and we defeated Belial but Belial or both could be easily reincorporated for a total of 7]) to unlock the secrets of Tathamet (this could also be used as a lead in to another game through content patches).
There is definitely a lot left to explore in Diablo III, story wise, and if they truly want to close out the game that would be the way to do it. Another expansion could end with a showdown against a reanimated Tathamet as the three primes found a way bring him back. We could be distracted by the remaining two Lesser Evils, Duriel and Andariel. We could be tasked to bring down each realm, like we did to Azmodan's realm. After completion we would get and ultimate battle in the realm of Terror against Diablo/Tathamet (I am expecting Diablo to become Tathamet and realm of Terror is mostly empty [Book of Cain] so perfect final battleground).
I came up with this baseline in less than an hour. Easily a lot of content that could be added to the game or made into an entirely new game itself. It would be great to get a massive expansion especially considering all the time we have waited or large content patches over the course of the next few years with the promise of another game!
I doubt Blizz would spoil the new expansion title like this. T-shirt namesa week from Blizzcon? Come on.

Well, if they decide to fix paragon and balance all set items this is pretty much a must buy for every Diablo fan.
@Karasu47: They have plenty of time for the trademark. Also, it could already be registered, but not appearing online.
Heck, they could even remove paragon completely, at least at the start of the new xpac, and focus specifically on runeword like system as well as a skill system.
Would be nice to have modifiers that were supportive of the skill instead of just "do +x% damage"
Take the WD for example, instead of having Helltooth do 1400% damage or whatever it is they should simply have made Helltooth activate all Gargantuan runes while adding an additional zombie dog and gargantuan to your arsenal. The short mans finger should add 1 additional garg and not 3, and it should NOT increase the Garg power so much.
I get that they went the + x% route do to performance related to high level GR's REQUIRING massive density. So builds that summon a lot of creatures or produce a lot of animations caused extreme performance lag.
That being said, I took these old builds and tested them, such as the zombie dog build, pulling an entire map, and it would appear they did a TON OF SERVER updates to prevent the same lag, I actually never had to hit esc to clear the lag before spamming skills and hitting esc again (many of you will know exactly what I am talking about!!!).
What this tells me is that they very well might have the opportunity to again focus on creative skill design instead of boring and bland +x% damage. This is really going to make the game insanely diverse, and I CANNOT WAIT!!!

I am focusing on the claim that the expansion is named "realms of evil" and features the icon on that shirt. Sure, any of this lore could be a great expansion setting: but why would they choose THIS image with THAT fairly crappy name for an expansion, as they had already appeared in the book of Cain? Ruling that connection out is important, so people don't narrow down the possibilities too early in their line of speculation.
If past precedent is anything, an expansion name would speak to the main antagonist, as LoD and RoS have, regardless of where it was set. Dragon of Burnination, or something.

The t-shirt is a clear reference to something - whether it's the expansion name or the cursed realms feature making its way to patch 2.5 we'll see in just a few days. Not that anyone cares what the expansion name will be really, as long as it fixes the old problems and adds the value D3 needs right now.

I actually like this idea.
I now have two suggestions: paragon 3.0 and lloot 3.0.
paragon 3.0: you allocate a certain amount of points to each passive skill. You will not be bound to your decision you can still reset. Example: take Boon of bul-Kathos passive. currently its used for the 30 second off of WOTB. What if you can allocate points to it to make it 45 seconds max. currently in paragon 2.0 there is no diversity. You grind levels till paragon 800 and then all the points after that go to main stat.
Loot 3.0: remove the paragon system all together. Revise the base skills. I would even go as far as to remove all class set from the game. The Legacy of Nightmare was the best idea that came to the game thus far. Lets build on that. Lets get rid of the rings and have an in game mechanic in which you gain 100% dmg per piece of ancient and 4% dmg reduction. Now lets drop in runewords now. I'm a D2 noob and too lazy to do the research. All my knowledge is from D3 and other RPGs. runewords to me a little stones that can unlock a different potential. Currently, Gloves, bracers, boots, shoulders, and belt do not have anything you can socket into them. Lets change that. With a max of 3 runewords. you can unlock an extra rune.
We have still yet to see the Corrupted Druids that were shown in the artbook as well as the slideshows they showed at Blizzcon those many years ago, I can not locate the pictures

Diablo 3 was a 2012 game but hearthstone is a 2013 game at least. The sign was there i assume before hearthstone revealed.It was a small hint.The next day we got a hint via overwatch that cain lives.Perhaps in this year blizzcon we might see an expansion for D3 that has an act 6 in which a task is to save cain ... again. We clearly cannot save him, but we can save his soul, and maybe something could happen where Tyrael becomes an angel again, with a new body mightier than Imperius, and Cain can use Tyraels body as a vessel. This allows the Horadrim to rein eternal.
It would be a way for Tyrael to help keep the issues at bay, both demons and the nephalim.
Regarding the tweet, That looks a lot like one of the portions of a map in Act 2 where there are bone walls surrounding the path to the center where a chest is located. You open the chest and skeletons attack. However, I do think that these cain lives comments may shed light on the future of our game, and though I understand my scenario is extreme, I do not think trying to save Cain's soul is extreme.

This seems like Blizzard being Blizzard and leaking things on purpose to build hype!
