My internet connection is no where near anything bad, and about 70% of the times i die, is due to lag. 200 latency, and i stutter everywhere, sometimes its okay, but lately its been really bad!
200 is nothing...thats what the avrg Australian/NZ player has to put up with because blizz wont put servers over here when yous get your "200ping" we get thrust to 400-600[ifit goes over 600 I log off]...try playing with that, it is not fun
200 latency isn't bad. That's a fifth of a second server lag. Shouldn't be noticable, unless you're some sort of pro-gamer.
I'm no pro gamer, but 200 ms is a huge amount of time. In fact, 215ms is the average human reaction time according to http://www.humanbenchmark.com/tests/reactiontime/stats.php (you wait for a red box to turn green, then click as soon as you can. I averaged about 200ms).
On my demon hunter I get one shot when mobs charge at me from off-screen. Lets say they take 0.5s to perform the charge animation, thats a conservative estimate. On 200ms lag that leaves me 300ms to hit my trusty SS button. That means only 0.1 sec of unawareness = instant death. When I play at ~50ms lag there is a noteworthy difference; I usually don't die then.
To the right on your action bar is a vertical colored bar which is green-yellow-red depending on the state of your connection. Mouse-over it to get the latency
Playing FPS games, like CoD, a 200+ ping you will die a lot by those with a much faster ping just because of the way the game's engine is designed, plus it is a twitch reaction game. However, from what I've found in D3, if your ping is under 280-300ms you can play the game just fine *IF* there are no lag spikes. When those happen I also have died. Ideally gameplay is great for me early in the morning (east coast of Canada) when everyone else in North America is still in bed, I get a 80-120ms ping and the reaction is really noticable. But evenings and nights the latency crawls up there, with some lag spikes. But, there have been fewer of those lately either due to Blizzard fixing their server structure, or the rush to play the game has died down somewhat and putting less demand on the servers.
tbh you should maybe change your ISP or at least check your equipment, I notice a lag spike maybe once an hour, and I don't have the best connection as well (Israel to Euro servers).

Usually in other games your ping is low because It's just seeing how long it takes for info to be sent to you, but Blizzard accounted for how long it takes for you to send them then for them to relay it to you. thus you see more ms in the latency bar.
I never even noticed the latency bar cuz mine is always red. now since i know it is there, i think the lowest i've had is about 300 ms. it spikes over 2000 occasionally, which is actually inaccurate cuz i'll have times where nothing happens on my screen for 15+ seconds! (I will admit that i think this is mostly my internet problem). still frustrating though.
Latency Indicator The latency indicator in-game is not a simple ping like most games, and is actually a full process of the game sending an action to the service, the service processing it, and returning it to the client. This means that the latency indicator actually gives a more accurate account of what the experience is for game data, but in comparison to other games will seem high. We're more interested in issues where there is actual performance degradation, and certainly there may be, but the in-game latency number is not a simple ping and therefore shouldn't be used literally as a measurement against performance elsewhere.

however, on topic im always sitting at around 100-250, but get no lag at all.
