I haven't spent much time with Revenge in the past, perhaps because I never understood how it works. Can someone explain how they use it in their 'rotation'? Do you have to watch for the icon to light and then press it? How long do you have to use it after it procs?
I play on an laptop, so the distance I have to look down is small. If I don't see it, I can quickly glance at the skill bar or try spamming it. It depends on the situation, I guess.
Mostly, it's quick glances to the skill bar to see if it's ready or not.
If I don't need the health, but won't need it in the near future, I use it for the damage.
If I don't need the health, but might need it in the near future (playing it safe with potentially-dangerous packs), I'll save it for when I need it. If I gauge that the pack isn't dangerous enough to warrant saving it, I'll just use it whenever for the damage.
I see it light up in the periphery of my vision and I press it... that is all. Ok, if I need the health I may save it if more mobs are coming at me soon, or if I'm about to jump into another big pack, but pretty much... you just press it.
I have it bound to my left mouse button. I tend to use it after using furious charge or leap into a mass of monsters and spam revenge. I use my peripherals to see when the button lights up. It works very well for me I must say.

I find it the most essential skill in my entire build (I'm a tank barb with [buffed] 25k dps, 45k hp, 900 all resist, 9300 armor). When I get into tough situations the constant large heals from revenge are what keep me alive.
