They are not results of levels (except for Inferno, u'll prolly need to have both completed Hell and be lvl 60), this is kinda opposite.
Though the best way to understand is to play D2.
Once you beat the first difficulty normal mode then you move onto the second - nightmare. After you beat nightmare then hell mode. After hell mode inferno mode.
In addition to the four different difficulties, keep in mind that the difficulty also scales (to a lesser extent), depending on how many people you are in a group with. So playing with a full group of 4 players, will be different than playing by yourself. Although playing with four others scales the mobs to be harder, you also have a better chance at loot and getting through content quicker because you are with other people, and not npcs (Followers). Not to mention, most people find playing with other people to be more fun anyways.

My question to you out there is how does difficulty work? Does it work like this:
When you are level 60, do you form a group to run dungeons at different difficulties for gear and runes? Or do select a difficulty when leveling up?
I would most happy with someone explaining this since I can't find anything that would explain how it works.

Well how it works is : you beat the game on normal then you can either select to go back threw the game on normal or further advance into nightmare. Then after nightmare theres hell...then inferno
they saw when you finish normal you'll be around 30
when you finish nightmare around 45-50
hell 50-60
So its going to be interesting lvling process for sure! I cant wait for inferno!! I hope its super hard!

I'm guessing its quite similar to the way in worked in Diablo 2, when you beat the last boss of a certain difficulty the next one unlocks, in Diablo 2 you could create games for the lower difficulties as well, but you could only see your current difficulty games on the available game list.
