Ladder means your character will take part in the seasonal level ranks, both the general level rank and the class specific rank. If you're one of the hundred highest level players, your character's name will be displayed at the ladder board.
Other advantage includes items that only appear for ladder characters. The "disvantage" is that the supply of items in ladder economy is far smaller so it's harder to trade and obtain the items you want (but many players see this as a good thing).
Ladder is basically in season and Non-ladder is out of season so any character made between 2000-2011 whenever last D2 patch was implemented plays in Non-Ladder. New Ladder season would come out once or twice a year and everyone playing ladder would get to start from lvl 1 and no items again and anyone Non-laadder would remain Non-ladder while those who had been ladder would become non-ladder characters, and any character made after the patch would have the option of being non-ladder. I would recommend playing non-ladder because i would gues at this time more people play non-ladder as the game is over a decade old and not too many new players joining all the time. Also you would probably be able to land free items and people willing to help you, good luck... I am not sure why i kept capatalizing the (N) in Non-ladder... im still doing it... Non-ladder.
Ladder allows you to compete for score against other players. ie: you are ranked in order of how many baddies you have killed in comparison to the next guy. Ladder also has some specific items you can't get non-ladder.
Really up to personal preference which way you go. If you want to see your name on a leader board and maybe get some unique high-end items (hard to get) play ladder (but be aware of the ladder resets mentioned by other posters). If you just want to play worry free and just go easy go non-ladder.
well, someone told me they changed the ladder reset from each year, to every 6 months. and that the next ladder reset is due in march. dont know if its true, but something to keep in mind.
(when ladder reset, all ladder characters are converted to non ladder, to start the ladder anew)

I wish you would have posted sooner. I started an account when the ladder reset and gave it away to some random person last week (decided I needed to get some work done before D3 release). It was a pretty stacked account to.
