For instance. In one scene Leo is interrogating a women that is a resident on the island. Strangely enough, she seems sane. Leo carries on a competent conversation with her. However, during the conversation, she has a glass of water. Drinks from it,and then it disappears the next moment and she is cupping air instead.
How does that tell us anything though?

The movie, is from the point of view of Leonardo, the protagonist. He is insane and has a obscured perspective on reality. Since we see the movie from his perspective, we are given hints as to his sanity through the use of various incidents such as the one I described and also from the several hallucinations he has throughout the film.
These hallucinations could have been interpreted as induced by medication, but this is clearly not the case because in the end of the film the Psychologist reveals to Leo several coincidences and seems to know exactly what Leo is thinking.
