I play with wrath of the waste set and bul-kathos weapons. I have double unity but I don't need them, I use Ignore Pain - Ignorance is bliss. I run this in conjunction with an "Obsidian ring of Zodiac" and CDR on my shoulders, diamond in helm and 10% from paragon points. With a levelled goguk gem I achieve permanent ignore pain and near permanent zerker (maybe 10 sec downtime between each zerker). - You want around 47% CDR, I achieve this by having roughly 32% from gear and paragon points, and then goguk fills in the other 15%.
So far I've cleared a 51, I'm pretty sure I can go quite higher (but I've yet to try it) and my biggest draw back is damage. I don't really have survivability issues. The healing rune on ignore pain is in my opinion far superior to double unity. It's 50% damage reduction with insane healing, i rarely find my hp drop unless it's extremely hard hitting monsters or bad elite affixes.
