After defeating ONE OF THE TWO lesser evils (one still remains) the legions of Hell will be united and the civil war that prevented them from infesting the mortal realm will subside. The remaining lesser evil will unleash the legions of Hell into the mortal plain and Heaven will have to intervene in order to maintain the balance in the Eternal Struggle. So the front of the demon-angel war will manifest in the mortal realm and we will be inbetween both sides.
Who ever said Heaven wanted to help us in the first place? They just want to destroy Hell.

Why can't a heterosexual guy tell a heterosexual guy that he thinks his booty is fly?
Cain is really one of the two lesser evils (forgive me for not saying which one, I'm not super clear on which one has the better position in the civil way, but whichever one that is, it is Cain). Cain will tell the hero to go on a quest to kill the other lesser evil. After this is complete, Cain will vanish, and it will seem like the increasing invaders from Hell have taken him. Tyrael will take over as your quest giver, and will direct you to combat the invading forces at the front of the invasion point. As seen in the gameplay trailers, NPCs can be recruited to help. These will be mortals at first, but the further into Hell you go, they will be replaced by angels. There will be skirmishes with both demons and rogue angels, but the final boss battle will take place in t he depths of hell in some sort of throne room. Cain will be waiting there (think Phantom of the Oprah) in human form, and the hero will be ambushed by horadric magic. Battle ensues, and when Cain is almost dead he will do a transformation (much like Diablo's when you complete Act 3) and the second half will be against the lesser evil in demon form.
