I agree Don. It seems the major consensus around here is:
1.Mistress of Pain
2.Belial
3.Azmodan
4. Diablo (possibly combined with Mephisto and Baal)
5. Angel (Tyreal or Imperius) in an expansion.

well actually maybe act 4 could possibly just be baal since that old man took baals soulstone at the end of lod and possibly didnt destroy it but if im missing something and the old man did destroy and didnt use it and turn to baal then im thinking diablo or all 3 together for act 4 or maybe an Expansion but really expansion has to be tyrael at least

I third that. I have also believed in the fusion of the 3 Prime Evils as the game ending boss. Ever since I saw the 3 headed Diablo picture, as you stated before, just gave me this feeling. But I also think somewhere I came across something that said only Baal and Mephisto fuse together...I'll have to check around for that. Anyone else see this?

well why would just baal and mephisto return and the middle head is diablo? i mean it could be like leah's dreams or thoughts or just memory or diablo's spirit could be haunting sanctuary and somehow could return you know it actually could just be lava and rock on his shoulders looking like there is 3 heads but it would definitely be awesome with the 3 prime evils fusing
I have never read anything about any of them fusing together. My theory is based solely on the fact that nobody knows what happened to them after their stones were destroyed and maybe they had to combine strengths to be released from where ever they went.

We haven't seen the Diablo's stone destroyed ?! And Tyrael is missing...

and baals stone
maybe that "meteor" that fell down randomly and hit the church was diablo/diablo's soulstone and thats how diablo returned and screamed in leah's face in the trailer lol
yep so im thinking that people are just scared from diablo's terror and i think that
Act I - Mistress of Pain
Act II - Belial
Act III - Azmodan
Act IV - Baal
Expansion - Tyrael or Imperius
Act IV has got to be baal, baal's soulstone was never destroyed baal left with his soulstone unless of course theres some cinematic that baal was actually destroyed but maybe after Tyrael destroyed the worldstone baal was weak so he hid for a while and after 20 years gained his powers back...
Ahhh, this is turning into the Diablo/Tyreal thread...So I'll re-post my theory here too.
Baal corrupted the worldstone with Diablo's Essence, much like a soulstone. When Tyrael shattered it, it was fused with him and eventually corrupts him over the course of 20 years. He is rejected from the heavens as the "harbinger of doom" meteor and crashes into the cathedral. But he can't be the Act I boss, so he must escape to Act4 somehow (Like the wanderer in D2). It could explain why Belial and Azmodon are now in sanctuary now too, the more powerful D has reclaimed his throne.
Act I - Mistress of Pain (King Leoric as a mini-boss)
Act II - Belial
Act III - Azmodan
Act IV - Diablo/Tyrael fusion
Expansion - Imperius or something so cool we don't even know about it
eldius your avatar kinda looks like a corrupted tyrael just maybe red angel wings or black
and anyways that explains the meteor going down that, that is tyrael being expelled from the heavens wow that could actually be it!
but how it crashed down deep into the ground...act 4 might be like a lava setting again lol because if you noticed Diablo 3 has kinda been following Diablo 2 by act designs
Yeah it will be interesting to see if they use the same level types from D1, dungeon, catacombs, lava caves, hell-type. But it could just be the fire from the meteor, it's kind of hard to tell at this point.

o i found it http://www.youtube.com/watch?v=ouq5-TQH7R4&feature=related
http://www.youtube.com/watch?v=pDs0pgh3Ai0&NR=1 lol and if they actually made baal that powerful in the game it would be impossible
Not as the 3 headed mosnter, but a separate monster all together from Diablo. I know I saw this somewhere and I am on the hunt for it now.

No I mean that the 3 headed monster IS Diablo and that Baal/Mephisto could be a separate monster from that. But I'm dumping this idea anyhow.
Sorry I don't see where the stone is destroyed ? We just see baal leave with his stone !
