5 Areas, various regions for each: Crystal Arch - Capital city of the High Heavens. Throne of the Angel Lord High Heavens - Home of Peace. Halls are numerous, as well as courtyards, gardens, and plazas. World of Sanctuary - Home of Mankind. Deserts, jungles, forests, mountain ranges. Numerous caves and encampments. Burning Hells - Home of Evil. Rivers of lava, halls made of brimstone and fire, skulls and bones line the floor. Hell Forge - Capital city of the Burning Hells. Since Diablo II, the Hell Forge has undergone major reconstruction and is now fortifide more effectively by the minions of the Three. It is now considered the capital city of Hell, and it has been isolated from the other parts of Hell, partly to hide it from Azmodan and Belial's unending conflict. Thrones of the Prime Evils.
ACT I: Angel Throne... The place where all the angels live and the whole point is to kill some corrupted high-angel who is killing everybody. The Angel King should be in the town and he should be like a healer and stuff. The whole act should be in light and the closer you get to this corrupted angel, the darker it gets. The mercs should be warrior angels that fight sorta like Tyreal, except for they're weaker.
ACT II: The Demons of Slaughter... This should be the crossway between Earth and Heaven. The boss would be a really powerful demon located on Earth since its the lowest part of the act. The place should be more swarming with more demons and it should be pretty dark with the townsfolk walking around with weapons. The quests would be like finding/rescueing humans who are getting attacked by demons. The mercs would be humans, given by some guy who you rescued, that shoot light arrows at the bad guys.
ACT III: The Crystal Throne... This is Earth and almost at Hell. Earth would be in flames and towns would be burned down everywhere. The actual town you would be in would sorta be like the Rogue Encampment. the point would be to find the Crystal Throne and to bring light back to Earth. The throne would be, of course, guarded by the boss. The boss would be the like a spitit of evil who would raise the dead and swipe at you like crazy with each hit taking a lot of life. The mercs would be crosses between humans and beasts so they would be extra powerful.
ACT IV: The Abyss... This would take place in Hell and you would be facing really powerful demons and Diabo would be here. The town would be sorta destroyed and the townsfolk would be like surviving warriors who are now trapped in Hell. The act would be dark and in flames. Rivers of lava, pentagrams, and corrupted andgels, demons, spirits, and so on would be all around. Diablo's lair should be almost pitch black and demons would be swarming. Lava would be all around but it won't give too much light. Diablo would raise demons and he would be four times more powerful than the Diablo in D2.

I would also add that each act doesn't have only one town. At least three more in each. The first act would have 20 quest and each act later as 5 more than the previous. The quests would be harder and longer. The expansion would add more quests and townsfolk.

Sometime's I think I'd like to see DIII stick to the hack'n'slash overhead view that's made blizzard so much money in the first place..
But then again, I think it could be cool to see DIII played sort of like the latest "The Elder Scrolls" games. First peson view and all that shit. it could be an idea...but again. I'd like to stick to the whole overhead view, but with dynamic camera control or some shit.
Overhead gives you the feeling that you are the god of this world, like you can control EVERYTHING, and not just one really strong guy
First person gives you the feeling that you are the little bacteria in the eye of the main person, hardly being able to control him. Plus, first-person is just all-out lame.
