If you want to have max dps, you don't have many options with your gear. The only thing that varies is the amulet.

Head: Put a diamond into socket for more BH.
Shoulders: The probably only item you can get with CDR on it, so get it. As 4th stat you can go with Armor, AR or Life %.
Amulet: To get your hands on one with the recommended item stats or the suggested ones might be very difficult. To have one with an element immunity is very nice, but you can also go with a good hellfire or another which has the correct stats.
Torso: If you're lucky you get one with reduced ranged and/or reduced melee dmg secondaries.
Wrists: Strongarm beats everything. With up to 30% damage increase its the only viable option here.
Hands: Magefists are the only gloves with 5 primary stats. If you are very very very very very lucky, you get one with fire%, int, as, chd and chc and you can reroll AS to CDR. But the chances for that are very low.
Waist: WH gives definitely the most dps here, but you can go also with hellcats to have +elite dmg or the viglante belt for CDR.
Legs & Feet: Nothing special to point out here.
Ring 1: There's no way to go around SoJ. With luck you get one with CHC, CHD or socket and you're able to reroll int into CHC, CHD or socket.
Ring 2: You also have no other choice that go for a good RoRG. Best would be a roll with int, as and chc/chd/socket. Due the ring always comes with LoH, its not possible to get chc, chd and socket on it.
Weapon: Furnace will give you the highest dot-tick due to its 1.0 attack speed (which is unique).

Gems: You need someone which provides the +10% from the toxin gem. Normally the tank monk would wear it so you can wear BotP. BotT and Zei's are mandatory to maximize your dps output.

Core: Movement speed must be up to 25%. Rest goes into Int. Max Resource doesn't give you more dps but only more AP to start with. Due you do not regenerate more AP/second with more AP, its useless to put points in there.

Offense: It depends how your gear looks like to decide with which you go first. Try to aim for 1:10 ratio between CHC and CHD. The one that is lower goes first, the other second. CDR as third and AS are just wasted points to push your sheet dps.

Defense: Due we get much AR due to intelligence, go for Armor first. Afterwards its up to you to go with AR or Life. Life reg is the last you choose.

Utility: The only place where you're able to get RCR, Area & LoH are chosen afterwards. Due to the low proc- and tickrate of your spells, LoH is not that good as it sounds.

Build Guide

This build aims for 4 player groups which targets high greater rifts. Playstyle: Your tank keeps away the mobs from you while you stay at max range, casting hydra, BH and Blizzard. Use TP to get out of elite stuff.

Group composition: Tank (probably monk), 2 DH, you

Build: With EE you boost your whole party with at least 5% dmg which outguns Glass Cannon.
