Strategy
Teleport in, drop Venom Hydra, disintegrate them while they fight your decoys. For unavoidable hits from champions/elites/bosses, use Diamond SKin. Illusionist perk will keep your Teleport up to get you away from danger, while Force Armor will assure you won't get one shotted. Dodge from dexterity fills in the rest. Glass Cannon and Astral Presence perks will make sure you stay in the business of destroying things.
Notes
I am currently in Act II inferno and have had little to no problems. If you run into shielding mobs, you can kill them by kiting for a while. It takes longer of course, and you have to be quick with teleport/diamond skin, but you can do it.
A good way to MF with this build is to start a game right before The Butcher, build your stacks of valor up by starting on the Halls of Agony Level 2 waypoint and working your way to the Butcher. When you get to him, Drop venom Hydra and run like a bitch. Keep it up, and you will kill him in 2 minutes or less with the proper gear. Use teleport when he pulls you in and Diamond Skin if he's about to hit you unavoidably.
I used to use (or still mostly use) the same build except with Sparkflint instead of Teleport and Temporal Flux instead of Illusionist. I like it quite a lot but calling it "the best build" when you're only in Act 2 is a bit silly.
Also I'm not really sure what you mean by "teleport in" or why you would do it.
Teleport is the most important part of this build. Sparkflint is really minimal for this build and I would suggest against it. Also, I didn't call it the best build. I said as far as I could tell, it's the most effective for inferno. I'm careful to guard what I say.
Tbh, most of these builds seem a little too focused on the killing part. I just run:

Blizzard (Snowbound)
Energy Armor (Prismatic Armor)
Teleport (Fracture or Wormhole)
Mirror Image (Duplicates)
Diamond Skin (Crystal Shell)
Astral Presence, Evocation, Illusionist
Nothing really ever touches me (unless Diamond Skin is up and their attack will proc Illusionist), and Venom Hydra is really all the offense you ever need. Disintegrate requires you to stand still, and I just never will stand still in Inferno nor do I see the point in standing still. This is assuming solo of course.

I personally subscribe to the theory that only 1 offensive is underwhelming. Disintegrate speeds up the process DRASTICALLY. I rarely die with this build. I can't see where subbing Disintegrate for mirror images would be a good idea at all.

Technically I'm dropping Disintegrate for Blizzard and replacing your DPS boost buff with Mirror Images.
