I am not going to claim that this is fact because I don't have the math to prove it to you, but I've read that Life Leach surpasses LOH at around 100K dps.
Do keep in mind that the the life leach is reduced by 80% in Inferno... so 2% of damage converted to life is actually .4% of your damage.
Regarding gear questions, not assuming you're looking for legendaries, this is where you'll find some extra dps stats:
Rings: Crit Chance, Crit Damage, and Attack Speed
Neck: Crit Chance, Crit Damage, and Attack Speed
Gloves: Crit Chance, Crit Damage, and Attack Speed
Bracers: Crit Chance
Helm: Crit Chance
Weapons: Crit Damage and Attack Speed
I would've posted the max values for each piece, but since rings and amulets can now roll lvl 63 stats, I don't want to mislead you by providing incorrect info. I do know for sure that all pieces listed above have a max crit chance of 6%, except for gloves and amulet, which have a max crit chance of 10%.
I gave MP3 a shot in Act 4 today and no recipe, but I can't expect it to drop every time at that MP level. Those Inna's chests and legs seem ridiculously expensive though particularly good. I'd really like to get one.
To get an Inna's chest or leg piece with the proper resists you need is going to cost a fortune. I highly suggest the Blackthorne's chest and legs. and it's what I've been using. If you still want the Inna's 2 piece, you should get the belt and helm.
If you're bored OP, or anyone for that matter, try using Cyclone Strike, with the rune that gives it a larger radius.
That ability alone is the reason that I've stuck to my monk for paragon leveling.. I currently have a lvl 60 Barb and a lvl 60 Demon Hunter. Cyclone Strike is fun, amazing for grabbing annoying mobs, interrupts key elite abilities, and you don't have to use the teleport punch that most everyone uses that can insta kill you.
They will reset typically if your res timer is over 10 seconds except if you have an NPC following you, like Leah. They reset based on a certain amount of time not being in combat.
Edit: Note if an NPC is in your group (Leah, etc) and you don't get back to the pack in time, they will reset as well.
1. Movement speed is excellent and never hurts, but isn't necessary
2. I personally disagree with glass cannon builds, but it works for some people. I guess it really depends on your sanity :). If you can take running around and some tiny frog leaping from off your screen to kill you, then glass cannon will work for you. The general concensus is basically "Don't get hit." It's kind of dumb and blunt to put it that way, but with no real damage mitigation other than smoke screen native to our class, it's sad but true.
3. For champion packs it's all about how good at kiting you are, avoiding damage, and using your discipline effectively and efficiently. Preparation is also required in my personal opinion.
4. What appears to have been the most successful for people is stacking Crit damage and using sharpshooter as a passive to burst things down, with a green gem in your weapon. I've read using sharpshooter is a straight DPS increase, but I have yet to see math on it, but I'll trust the community's opinion.
Sadly I need a new weapon because frankly my DPS just isn't high enough to deal with some packs, but with those combination of talents I've had no problem kiting things forever.
Ive probably logged more hours than that... Been farming Inferno Act 1 and I've seen 1 Blacksmithing pattern drop and no legendaries. All RNG my friend.
When mobs have reflect damage I generally spam my Hate spells (currently I'm using Multi-Shot with Fire at Will and Elemental Arrow with Frost) with SS up. That way your large damage dealing spells aren't the ones reflecting back to you. This is also pending on whether or not their other affixes allow this and only having to use smoke screen to mitigate the reflected damage. Other than that you just have to kill them a lot slower with your hatred generating spells.
Yesterday I just burst out laughing when my combo on those super fast frog things (I'm a Demon Hunter) was jailer, frozen, waller, invulnerable minions. I immediately died.
Just finished Act III last night with another Demon Hunter and a Monk I've been playing with. I'm sitting at ~9K DPS and 24K health, and the other demon hunter has less health but is around 11K if I remember correctly.

Build wise I use this and has been working pretty well in Act II http://us.battle.net/d3/en/calculator/demon-hunter#aRYdXV!TeY!YcZYZb
