Rifts Guardians drop keys for greater rifts.

At the end of a greater rift you can upgrade your gems, if you beat the 15 mins time mark.

Upgrades have a chance of 100%, if the GR is 10 level above the gem, decreasing beyond that. i.e. 60% at same level.

You get 3 upgrades + 1 if you did not die + 1 if you empowered the rift by gold.

I think he means modded gems. It's mainly seen on consoles. You can't get these by any means within the game. You need to have them dropped by other players or mod a save file.
Yup, that's what I mean... so those gems are basically hacked. Guess that explains why I couldn't figure out how to make my Lgems like that haha. Thanks for the information =).

I have been playing online a bit on PS4, and I see legendary gems on other players with insane amounts of damage added to them. How is this done? Kanai's cube seems to only add stats according to gem type (int, str, dex, etc.) and it doesn't seem you can enchant legendary gems. What am I missing here? How do you add the massive damage to legendary gems? Seems like this is the only way you can do anything in the higher level rifts.
