Stating the obvious is how easy it would be for them to macro out higher resolution art for Diablo II, but would require an update to the engine to handle it:
Not if they do something like try to use an over-the-shoulder camera angle or make a generic pay-to-play diku clone or poop out a turd like Hellgate: London.
The ideal Diablo III would provide the ideal Diablo experience: more of the same, higher resolution art, and a system in place to monetize the production of ongoing content, whether that's a Real Money Transfer or whatever.
Myself, I think the Sin War would be the ideal setting for it, but meh.

There are still some people.
Hellgate: London is maybe a sign that maybe the management wasn't too essential in the end?

That screenshot would be a pretty tacky way of denoting it, unless it's something toggled with a button press.
Mentioned in another thread, Zerg units upgrading into their palette swaps (Hunter Killers, Devouring Ones) could be kind of cool..

Even some new uniques would be kind of a big deal.

Titan Quest would have been more successful than Titan Quest if it had two dimensional art.
Titan Quest would have been even more successful than Titan Quest if it had a real online mode.
