If Cain were to die, his secrets would be left behind and they'd probably answer a few questions, aside from assigning the next big quest after Sanctuary is caught in the war between Heaven and Hell.
It would be nice if you could choose the gender of your character and have several means of customization at your disposal prior to creation. As for changes to already existing character archetypes, what they need to do is expand skills trees a bit. Regarding NPC's, I have no doubt we will see familiar faces in the third installment, however, what time period we enter the realm of Sanctuary is still up for debate.
The Sin War novels were a decent read, however I don't believe it was one of Knaak's best works. His stories that involved Norrec or Zayl were much more entertaining.
