I think the Worldstone did both hide Sanctuary and shield it from Demons and Angels. However the latters are still able to enter Sanctuary because the Worldstone isn't perfect and it has apparently lost some of its powers for mysterious reasons.
And also, could it be that the barbarian bloodline is just incredibly strong. It might not necessarily be nephalim powers. If the barbarians are the sons of Bul Kathos then that lineage alone may be strong enough to defeat a prime evil considering how brutal BK was.

um..you do know that he didn't manifest his powers himself right? Thats why they erased everyones memory because they were not ready for it. Lilith was the cause of all of that.

But to me, those powers would take a very long time to manifest themselves and Diablo 2 is barely any time past the Sin War because from my perspective, "The Dark Exile" and Diablo's takeover of Tristram all happened very quickly. But the Sin war itself lasted for a long time according to lore, but for how long we don't know.
First, the Sin War is the war between Angels and Demons to get the souls of humanity, it never ended and it's still going on.
Second, the events of Diablo2 happened about 3000 years after the Sin War trilogy, and there's several hundred years between The Dark Exile and the events of Tristram.
Third, it took Uldyssian only months for his power to fully manifest which almost destroyed Sanctuary and he is just a simple farmer Lol. Maybe D2 heroes aren't technically Nephalems, but they are at least imo the beginning of a new generation, and with the destruction of the Worldstone any humans will now be able to "unlock" their Birthright with just a little help... just a little.
There's no specific explanation but I think that this alteration is somewhat responsible for the creation of the Horadrim and those D2 heroes who can kill all three Prime Evils at once with their bare hands...;)
Still I believe the alteration wasn't absolute, so the Worldstone still prevented the Nephalems' power to fully manifest itself... until it was destroyed that is.

Every humans can be a Nephalem, however apparently the Ancients aren't part of the firstborn, they are just humans whose true powers were unlocked.

He was only 19 when he was called to serve the Ancients. Before his calling, he had only heard legends of Mt. Arreat.

He performs miracles on unfortunate people (like healing the blinds and so on) to manipulate them into joining his religion at the cost of their soul. His aim is to entirely replace the Church of Zakarum and rule over all of Sanctuary.
The last church he built is still there in Bramwell, without doubt one of the biggest church in Sanctuary and it has probably been abandoned a long time ago.
