I've searched this topic a bit over the last week and have tried multiple things
I'm just minding my own business running around with a couple friends slaying some demons and doing bounties while everyone else rifts since i cant seem to keep a steady supple of keys on hand... (ya know what else are you supposed to do with your friends right?) at about 45-50 fps. Every 10-15 seconds my frame rate drops down to 0-5. I can't figure out what is causing this to happen.
I really doubt that my system is the problem as I'm playing on a shitty Alienware.
I've tried a lot of things like turning off damage numbers, checking my background applications and so much more.
Wondering if anyone else is having an issue like this. It's almost unplayable and really takes the fun out of it.
If you have any suggestions, I'm willing to try anything at this point.
D3 has a poorly designed engine (IMHO) try lowering your sound channels, set the shadows to low (not off), turn the audio off (some ppl says it helps i haven't noticed it) and turn off every display number thingy.
I still have < 30fps most of the time and in G50+ it plays like a powerpoint but my specs aren't the greatest.

Control + S ftw
Settings to lowest possible, all numbers off, problem still persists - have not tried with sound channel turned off, will try that tomorrow.
