Regardless of its strange appearance, both the image and the wanderer share two striking similarities. Both had a hood over their head, and both had a soulstone cavity.

Come to think of it, we have no proof that this thing has arms (and we have no proof that it does not). Of course, IIrc, in D2 Diablo would occasionally move on all fours, which might make this more of a lunge.
Now moving on to Bloodlust Fox's comment, why is it everyone assumes that if it isn't Diablo it's Belial? For all we know Belial actually looks more like a centaur, or a raven, or a jellyfish for that matter. Furthermore, saying it looks like a balrog, I always thought Diablo looked like a wingless balrog (this was before LotR came out and I saw it in it's firey glory), so why should that have anything to do with it, come to think of it, he does look a little like a wingless balrog as far as Diablo Balrogs go. It's not to much of a stretch from D2 Diablo to this guy. It's no more of a stretch than it was from D1 to D2.
In the mean time, I hearby challenge anyone on these forums to come up with an alternate explanatioin as to what that mark on this guy's forhead is. I think it's from where the soulstone was. I challenge people to come up with a sound alternative. I will be impressed and influenced by nothing less.
So far it's been last boss on the cover. Diablo, Diablo II, and Diablo LoD stay true to that.

So far I really like it, I don't see how people are honestly saying they think the old diablo looks better.
I'll try to explain it.
Essentially, human beings are capable of forming things commonly known as "opinions". You have the opinion that the new Diablo looks better.
Now here comes the really hard part.
Some people will form different opinions to those that you hold! So in this example, many other humans (including myself) hold the opinion that the old Diablo looks better.
1.) In the Tombs of the Horadrim, the remnants of the Wanderer's spirit and will was crushed by that of Diablo, forever suppressed.
2.) The corporeal manifestation of Diablo was derived from the body of the Wanderer. This form was destroyed. Therefor, while Diablo's spirit remained within the Soul Stone, his corporeal form was lost, demanding he acquire a new one. He didn't. Ergo, the Wanderer, in both spirit and body, is lost.
The Soul Stone likely left an indelible mark on Diablo's very spirit, a gaping wound that will never heal.

Diablo himself has never been "pierced" by his soulstone.
Diablo was bound inside the soulstone. The only things that have been pierced by the soulstone are his hosts. This is where the scar would be only. Diablo has no scar from his soulstone because his essence has always been inside it since he was bound in it by the Horadrim until we destroyed it on the Hellforge.
We only see Diablo with a soulstone jutting out of his head because what we see of Diablo is his spirit completely overtaking a host which must have the soulstone thrust inside them. When the soulstone enters a host Diablo can slowly spill his essence out into the host to begin corrupting it. (we know this takes time) Once he has full control over it, he may change the body to resemble his true form. But what we are seeing is still the host with the soulstone in it.
So no...Diablo would not have a scar on his "spirit" from the soulstone.

Note that it says "altered". He changed the look("form") of that child to look like his body. If what you said is true it would have said that he shed the child's body and came into his own.
Where the hell did the three heads come from? all i can see is one head, a longated demon figure with two eyes a nose a mouth and holes in the jaw., and it sure looks like a re-vamped and uber bad-ass looking Lord of Terror, to me.

Honestly, on this one, I somewhat lean more towards Archanum. I don't see how Diablo's soul would recieve a mark from the soulstone being driven into his hosts head. That said, I won't rule it out either.
My theory is that it's the Dark Wanderer's old body (and yes I realize that the wanderer we once played as is most likely gone forever) ressurected, and Diablo's essence, or at least a portion thereof, is inside that body, or at least tied to it, so that if the body were ressurected, Diablo's soul would return to it. It's a stretch, but here's my reasoning: When the Wanderer took down Diablo, the body reverted back to the form of Albrecht. Now when the stone was removed from Tal'Rasha, Tal'Rasha was still fairly posessed by Baal and did not revert back. In fact, even when he took the stone back, he never stuck it back into him, but rather wore it around his neck. Evidently, at least some of Baal's essence was still in Tal'Rasha even without the soulstone. What if Diablo did the same with the Dark wanderer. He kept it in the stone with Albrecht, as the prince was not the host he wanted. Once he was in the much more suitable host of the Wanderer, he took up a more permanent residence. Now as far as we know, when Big D wend down in D2, the body did not revert. It might have, but it might not have, and we never saw it do that. Now if that body were rezzed, it would probably be enough to call back Big D's soul from the void it was sent too. Just a theory. Nothing more, nothing less.
EDIT: I just watched terrors end, and I two fatal flaws in my theory. 1. The body exploded. 2. you can visibly see Diablo's soul (which looks exactly like Big D, for all you who contend for the the true form of Diablo theory) get sucked back into the stone. I still think that this is Diablo, but my theory's sunk. Oh well.

I believe that the reason for Tal Rasha was because the stone was broken and incomplete so the Horadrim used Tal Rasha's body as the other part of the soulstone. So when one part (the actual stone) was taken out, half of Baal's essence was still inside Tal Rasha. This explains why Baal needed to get the other half of his soulstone, so he could be complete.
I thought Tal Rasha had become corrupted himself by containing the soulstone for so long.
Isn't that why people started putting them into themselves in the first place? To keep the corruption inside them and not let it spread to other people and surrounding areas?

Now, for Archanum, as iIrc, Big D's soulstone was shattered by Lazarus (talks about this in the D1 manual), and it was a shard that the wanderer shoved into his brain, so that actually helps my theory.
Whether it is Diablo or not, I am not sure, but I have noticed something. If you take a look at who ever that dude might be, you will notice a opening or a gash if you will, in the forehead...I do not see a soulstone but it looks very similar to the mark the soulstone leaves when taken out. Just something I noticed, I am not trying to prove that it is Diablo.

Sorry, should have responded to this earlier. I don't exactly buy it, but it's probably as good as anyone can get before more information is released.
