yea, going along with what XxL2PxX said, in both Diablo I and II Diablo spawned through a human. Took over their body and grew from the inside out so to speak. So perhaps the image shown is nothing more then a more natural form of Diablo. Diablo unrestrained by the human body, or a true form.
and anyways Diablo isnt re born he is simply sent back to hell. so he just has to somehow get back to sanctuary. but for the last 20 years i would surmise that now that the prime evils are back in hell they are fighting to regain it from Belial and Azmodan

I think it would be fairly reasonable to assume that this is Diablo's "new look" because of the gash in his forehead, which was likely once filled with his soulstone. Of course I could be entirely wrong, but anyway, we'll see.
It doesn't really look like a gash though. I think it looks more like decoration.

Or maybe the gash has just healed somewhat? Or maybe its a combination of decoration and healing, because that demon's face has nice intricate designs on it as well as being a ferocious skull.

I will say this though, I absolutely loathe the idea of the Three melding into one being, that strikes me as being a very cheesy attempt at upstaging the previous games. It would also probably mean a story something along the lines of "the Three intended to have their arses whumped and planned to come back as one crazy uber-demon of doom twenty years after Tyrael blew up the worldstone all along!...Honest!".
As cool as the concept of Belial and Azmodan is, I feel that making a whole game (in a series titled "Diablo") starring them would be akin to making an "Indiana Jones" movie where Indiana died of prostate cancer ten years before and his son continues his work, and the skull we see looks a little too Diablo-like to be anyone else (the known Evils seem to be pretty diverse in appearance). I think it's likely that Diablo intended for the Soulstones to be destroyed (he hated the things; both he and Mephisto tried to get their servants to shatter them). The destruction of the Worldstone may have also destroyed the barrier between the mortal world and the abyss which contains the souls of the Prime Evils, or something occurring somewhere along the events of Diablo III may apply a similar effect.
Recall that, at the end of the first Diablo, Diablo's body turned into a big skeleton before collapsing into Prince Albrecht's body once his soulstone was removed. In Diablo II, we see a blue matter encompassing Diablo's body enter the soulstone, turning his skin black (which didn't happen in Diablo I, probably because Diablo hadn't fully manifested in the boy's body without the reunion of the Three), but we don't see the soulstone removed, nor the body collapse into a skeleton and then the body of the Wanderer. Perhaps the hero removes the soulstone, then hurries off to the Hellforge, not noticing that some surviving demons preserve the skeleton, and try to take control of it? Maybe what we're seeing is the skeleton of the Lord of Terror, partially restored and being controlled by one or more of the Lesser Evils?
Coincidentally, I'm not sure if this has been thought about, but, relating to the idea of Diablo planning all these events and the recent "and the Heavens..." teasers, perhaps Mephisto's prophecy represents the events of the various games?
"And the Evil that was once vanquished shall rise anew!" (In the opening cinematic of Diablo I, the evil that the Horadrim thought locked away permanently (Diablo) awakened.)
"Wrapped in the guise of man shall he walk amongst the innocent, and terror shall consume they that dwell upon the earth!" (In Diablo II, Diablo traveled in the body of a man, walking among the unknowing populace, and terrible creatures manifested in his wake.)
"The sky shall rain fire, and the seas will become as blood!" (The skies rain fire at the beginning of Diablo III, culminating in a comet-crash where Diablo once slept. The seas becoming as blood could refer to a development later in the game.)
"The righteous shall fall before the wicked, and all creation shall tremble before the burning standards of Hell!" (Perhaps Diablo IV, a big expansion, or even the final act/ending of Diablo III? It's interesting to note that "all creation" was altered to "the heavens" in the teaser statement.)
This version of Diablo (yes i think it is Diablo) is much more original than what we had all hoped for in the back of our heads.
The previous Diablo, no matter how cool in it's own right still was a combiantion of everyone's popular view of what a demon looked like.
This version on the otherhand looks much more like a Prime - evil to me. A great big motherfucker that gives birth to destruction. Wouldn't want to face that son of a b even in my dreams.

in reguards to your post, mephisto was speaking of Diablo finally coming to his true self from the warriors body, not neccesarily his true form showing his full POWER
and from reading this site for many yearsi thought it was confirmed through lore/books that the soulstone confined only a portion of diablos true power, so he is able to appear in the mortal realm.
now that the soulstone is destroyed Diablo can no longer appear in the mortal realm but he can now have his full power express.....if indeed the worldstones destruction always him to enter the mortal realm freely.
Well, comparing the central head to the face seen on Blizzards current splash page, yeah, it's Diablo. As for the other two mouths, he's just looking more powerful and cooler. I seem to remember something in the Black Road about demons and angels taking the form they choose, mostly to inspire fear. Is it unreasonable to think that The Lord of Terror is capable of changing his form to make himself more terrifying. Besides, it wouldn't be the first time Blizzard has changed the art on this sort of creature for a new game.
Somehow I doubt the other two mouths belong to the other primes. They aren't uber enough, and they're just mouths. They'd need to be more. They're probably just really nice shoulder pads that, being attached to a Demon Lord, are posessed.
As you can see below, Diablo's horns and general features are different depending on whom (if anyone) he's taken the body of.
On a side note, I'm glad Blizzard is returning to some of its old funky camera angles with the cutscenes. Diablo II was epic at many points, but it lacked the pure roller-coaster factor that Diablo's cutscenes had with the fast, spinning and surreal zoom-outs.
to me, diablo in the middle seems like the balrog in Lord of the rings. And can anyone tell me where you've seen diablo3 diablo pic?
Haha, I think Deckard Cain grew a beard so he could do a "YOU SHALL NOT PASS!!!" cutscene with Diablo.
