It's my theory that Leah is experiencing a memory, not just a vision, from the occupant within the crystal (possibly Tyrael). That is to say, I think an entity, possibly Tyrael, resides within the crystal. And it is their memory of an encounter with the prime evils that Leah is experiencing.
Asmodan seems to be addressing someone who had planned for the Evils to fall, one by one. Perhaps Tyrael had forged a pact with the Lesser Evils to have Mephisto, Diablo and Baal exiled to Sanctuary, where they could be hunted down and imprisoned? Because it wouldn't make sense for Asmodan to accuse Leah of this when she's never met any demons before. (Well, assuming she hasn't.)
As for the demons on the pillars, I think they are the new Lesser Evils as appointed by Asmodan, of which there appear to be seven. Also, any idea what that dragon-like creature is in Cain's tome? Perhaps Trag'oul?
Another observation: I think you can hear someone cry "Mercy!" right before Leah has her vision. Sounds like Deckard Cain. Is HE trapped in there? You can hear it at 1:14.

I remember those images. I thought that they were just concept art for Azmodan or belial, but looks like they are Azmodans generals (I guess). I love how the cinematic confirmed one thing, who Azmodan is, but presented like ten more questions.
Maybe the Blacksoulstone is a lost part of Anu with some essence of Tathamet and whoever can unlock its powers becomes the Prime Evil (We know Diablo becomes it)

How might we reconcile this seeming discrepancy? Easy.
I propose that Deckard Cain is Belial, Lord of Lies. He's been using us to destroy his enemies over the years, and his plan is almost complete.

If Diablo was inside Leah at that point he'd probably smite the fuck out of Azmodan, excuse my language. Diablo is a prime evil, Azmodan is a lesser evil.
It's more likely that he's referring to Leah listening in on their plans. Chris Metzen also noted that some scenes had been cut out of the cinematic to avoid spoilers though, so who knows.
Doubt it Dae. Clearly if Diablo was inside Leah, he had not been able to completely take her body. Remember how long the possession process was for the wanderer? It takes time and Diablo is limited by his hosts for some time.
The most plausible suggestion according to lore is that he is talking to Diablo inside of her.

Diablo only possessed The Wanderer through the soul stone. Leah doesn't have a soul stone inside her, at least currently. He may be influencing her but he's not possessing her. The only other case of clear possession was when Lilith hopped inside Serenthia's body and used her like a suit, which required Lilith to have a physical body.
Well if it's in Hell I guess the thrones could be the original 7 evils and since most of them are gone Azmodan decided to set up shop and play boss down there.

Since the heroes of Diablo 2 were the ones to do this, the person Azmodan is talking to had to be manipulating those heroes and also have a connection with Leah. All signs currently point to Cain but there are also other possibilities.

Before the original 3 prime evils Mephisto, Baal, and diablo where banished to sanctuary they had lesser evils (perhaps appointed by them) this is likely the case anyway. Azmodan and Belial where the remaining lesser evils in Hell and as we know and fought for control of Hell, once that the 3 prime evils where trapped on the mortal plane. The thrones seen on the video could just be the appointed lesser evils of whoever is now in charge of hell (which is most like azmodan) by the sounds of it. leaving Belial perhaps a second in command to Azmodan who is already out in sanctuary looking for the black soulstone. After the events of diablo 2 the 3 prime evils are destroyed (or so it would seem) and Tyrael destroys the worldstone.
The black soulstone could be a combination of the original 3 prime evil stones, which holds the power of the 3. This could be why Azmodan is seeking it. Or it could have been formed once mount arreat was destroyed which is where a huge crater is.
Once again I don't know.
Anyone else getting the feeling that the lore intensity level of Diablo 3 has raised 100%. Since Blizzcon started

Can't it be just a random poor soul that sinned too hard and is getting punished in hell now? I feel like Deckard Cain isn't the kind of guy who would go "oh please, don't kill me" on huge demons, he knows what they do and he's wise not a coward like Abd-al-hazir (or whatever his name is)
