Only seconds after the destruction of the World Stone the high powers in Heaven felt the effects of the stones destruction.
Tyreals role was to preserve the World Stone and prevent the 3 brothers from tainting it. Tyreal failed Heaven and for that he was banished to the mortal realm. However...
For the Hero who defeated the 3 evils, he won't be packing his bags and heading back to The Western KingDoms... No something far more sinister has risen up to the East. It seems the 3 brothers physical forms were destroyed but surely their souls live on -- even through the destruction of their soulstones. They have convinced the great mage Horazon to help bring them back to the mortal realm.
After Tyreals decent from the high Heavens to the mortal realm. The legions of Hell were being spewed forth into the mortal realm. Heaven is taking it's army to prevent further chaos and destruction. However it is a losing battle, with the help of 2 lesser evils the war is not in favor of the Heavens. Heaven has sent out it messangers to rally support and gain more fighters from the mortal realm. Messangers have been sent to all corners of the sanctuary. To the Scosglen Druidic Forests, to the Amazon Islands of Philios, Lycander and Skovos. Messangers have also been sent forth to Duncraig for mercanarys, Paladins and other warriors. Messangers were sent to the jungles of Kehjistan to gain the aid of Sorceresses, Necromancers and Assassins alike.
Though there is a place where each evil can only retain their former bodies are long forgotten and are the sites of even the most dangerous demons of all. These demons have no alliegence to Hell nor Heaven, their actions come from orders even the Horadrim couldn't capture. An evil so overwhelming the name of it has been long forgotten. Mount Arreat, The Guardians Tower and Tristram have all fallen to the power of this evil.

im slightly confused with this little bit are you saying their is a seperate more powerful evil besides the the prime evils or that the three have attained their original bodies and are more powerful than ever
Yes, that is what I am saying. It says in the story "The Horadrim even chased Baal to the East fearing what laid there." You never know, Blizzard can be tricky that way.
After Destroying the worldstone Tyreal was condemned to the mortal world for failing to protect the worldstone. His punishment is to protect Sancutary from the any new threats that might happen do to Tyrael's failure.
Twenty years have passed and Deckard is searching the monastery in Tristram a bright light falls from the heavens and lands in the spot where Diablo once terrorized the world all those years ago. As the dust settles Deckard see a stranger limping to the site. Deckard could sense the evil from this man, even in the dark he could tell this stranger has been twisted by some unknown evil. The dust clears and there stands Baal. the stranger walks up to Baal and hands him a glowing yellow stone. It is Baal's soulstone!
Deckard in disbelief remains and watches as these events unfold. How could this of happened. Then out of no where Tyrael arrives and a great battle ensues. Tyrael being weak from his 20 years of exile from heaven is overpowered and in Baal's moment of victory he shoves his soulstone into Tyrael's chest.
Deckard awakens to see Tyrael standing over the body of what looks like to be Baal. Deckard approaches Tyrael and they discuss that Baal's soulstone wasn't destroyed and with the worldstone gone all he needed to do was to gain strength to cross over to Sanctuary. Tyrael convinces Deckard to follow him to Mephisto's Jungle to use the Horodrim's power to permanently close the gates of hell.
As Deckard and Tyreal arrive at the jungle Deckard begins to realize that there is something wrong with Tyrael. On there journey Tyrael destroyed a whole village stating that it was controlled by evil forces. Upon reaching the gate Tyrael begins to change and new evil emerges. This new creature is neither Tyrael or Baal but a whole new evil with the combined power of Heaven and Hell.
As the ArchEvil was formed Azmodon and Belial came out of the portal to stop this new ArchEvil and preserve their rule over hell since the Prime Evils where gone. A great battle happens and Azmodon and Belial are defeated with ease. Using the souls of Belial and Azmodon the new ArchEvil resurrects Memphio. Then sacrificing Deckard and using his powers of the Horadrim the ArchEvil resurrects Diablo. Diablo and Mephisto are not strong enough to take a physical form just yet. Without a soulstones to carry their power the Prime Evils agree to combine thier bodies into one. During the fusion a great evil is formed and once again Diablo is reformed stronger than with powers of Destruction, Hate, and Terror. Tyael's body is released during the fusion but the evil has corrupted him and now he sits at the right hand this new Great Diablo. Great Diablo enters Hell and regains his kingdom and prepares his forces to take over Sanctuary and eventually bring this battle to the gates of Heaven.
i'll say this for the above post: while creative, it fails to explain leah's purpose in the story, as well as define what this would mean for the world of sanctuary
i mean i love blizzard and all, but after 3 games in a series its unlikely they'll do another

Why didn't Deckard just use the waypoint to Kurast? Surely he grabbed it when I brought his ass through that way last time. lol
It is an interesting story. This whole "Tyrael is evil" stuff would piss some people off I bet.

The blind mind's eye replies with a sigh and a tear of contempt for those who see the world as it truly is.
Any slight miscalculation sprites from the fact that I know little about lore.
After defeating Baal, the worthy Barbarian traveled back to his homeland. There, he married the fair Anya. Their daughter, Leah, witnessed in her own eyes, the casting of Diablo, united with Belial and Azmodan, into the heart of Sanctuary. Such an unholy alliance of such a trilogy can only spell an ominous faith for our kind. Baal's faith is not yet known, but some say he's lingering the gates of hell, waiting to be set free, waiting to cast his revenge upon poor weakened Tyrael.
Tyrael is a champion amongt his kind. While he is the scorn of some, he is the icon of most. Yes, he destroyed the Worldstone, which was the only path to our own salvation. He has led a sorrowful life since then, but he knows he has a place within all our hearts here. He still dwells in the Worldstone chamber, waiting for another cause to use as an excuse to meddel in our problems. Some might say he will eventually grow corrupt due to the consequences spurred forth by the destruction of the Worldstone; most however, bare stupendous trust in that being.
Azmodan, Belial, and Diablo now stand as one, on their path to free the equally hazardous Baal, to make for an issurmountable evil. We can only pray and hope that you, hero, deliver us once and for all from this threat. Diablo's soulstone, child, reincarnated after the destruction of the Worldstone, though others did not, much like Mephisto's. We face a threat far greater than hatred, hero. The full consequences of the Worldstone's destruction are yet to be conceived completely. For what we know at the moment however, your aid is needed greatly here. Help us hero, help us bring back the ages of gold, and bring down this darkness into the abyss from which it crawled out of!
You can. I'm all ready. But I think you're an Egyptian, and I'm Lebanese..so two Arabs looking to dominate the world will only result in USA kicking both our country's ass.
Without a soulstones to carry their power the Prime Evils agree to combine thier bodies into one. During the fusion a great evil is formed and once again Diablo is reformed stronger than with powers of Destruction, Hate, and Terror. .
Well maybe heaven and hell are not so different in Diablo, my little guy noticed when you invert the Diablo III artwork suddenly Diablo looks quite heavenly.
I actually think that the name for Azmadon should become Azmodeous, because that is from Advanced DD, I know it is. And for another thing, if the 3 prime evils hatred destruction, and terror, well I think there should be one more, which if you think about it, would lead to the Lord of Doom (Azmodeous), and would be the most powerful being in the known universe. Just until the God of God comes (Aeo) and says nope, this ain't gonna fly. I think blizzard, or someone should work that into the story of diablo 3, or any orthers they will be making. Probably not though. Its just a thought. "And to the realm of Chaos, I herefore banish you, Azmodeous, the Ultimate Evil, and forever shall yo usstay there, and forever will you be tortured. And as for you Tyreal, you ahve done well. I cannot believe you would have done such a thing as help the three Evils, but you have been corrupted, and forever you shall be. For aiding Diablo and his brothers is an act like no other, and for this, you will stay and reside in the Mortal realm. This place will be your grave. This place will forevermore be a peaceful Sanctum." And so ends our tale, of the three Evils, and the Heavens. There is no more to tell, besides the remaining fact that Tyreal, an immortal being, left on Earth, will always be insane. This is just after the huge war between the Heavens, and the 9 hells. Aeo defeats Azmodeous, and as he lays there, unconcious, Aeo banishes Azmodeous forever to the chaos realm. (mainley because he was too powereful and Aeo didn't want to die trying to kill him.)
Hey, yeah this story from Amibeles seems like it should fit in the fourth game. Even though I agree Deckard Cain should have died like 100 years ago, I don't know how long a normal person lives in the Sanctuary lol.
