Sorry if I am stealing anyones thoughts, i haven't seen this written anywhere while i have been lurking, so i figured it would make a good first post. I am in no way arrogant enough to think anything I have seen, read, or heard is fact - so feel free to add, take away, etc...)
Diablo 3 (3rd of 3 games so far)
Demon w/ 3 heads in trailer

3 prime evils
3 worlds (heaven, earth, hell)
3 races - Angels, humans, Demons
3 elemental arts (fire, ice, electricity) - maybe stretchin it...
It occured to me, that the story of Diablo is one of balance. Heaven nor Hell has ever held dominance for long, for much of history in this world, there were only two sides, then sanctuary was created, caught in the middle, it became a pivot point - a point of contention to shift balance one way or the other.
3 perspectives
- the heavens want to maintain order... balance is a part of order.
- humanity - for lack of a better thought, is simply asleep for the most part, but when it's heros do rise, they choose to follow humanities own self interests (preservation) over either side it seems.
- hell - forces of chaos - they are the antithesis of balance, diametrically opposed to it.
taking a setp outside into the real world, we know that in judeo-christian beliefs, God is made of 3 beings, the father, the son, and the holy spirit. (could you perhaps rephrase these as a mentor, innocence, and purity) taking that as a frame of reference, and stepping back into the world of Diablo...
if we are to maintain balance - at least in the original sense (between heaven and hell) then there must be a heavenly counterpart to the prime evils, 3 goods... has it ever been clearly stated in the Diablo Lore/books that The prime evils have a father or mother? could it not be tha they are more properly thought of not as siblings (brothers as they are called) but 3 faces of the same entity? much as God is? Sanctuary need not have 3 of it's own 'heavies' as it is the center of balance - the pivot point, or the prize if you will.
So as many have wondered, is that 3 headed demon diablo? is it something new? is it the 3 prime evils united into a super being? does it have to be any of them? could it be more that they really don't unite into some super demon, but this is just THE Prime evil, not divided into three conceptual parts?
edit - forgot... you have the 3 evils - terror, hatred, and destruction, and the perhaps 3 prime goods? the opposites of them, the balance? a mentor, or father figure - shields you from terror. Innocence and Hatred can not abide each other, and purity is the way most things begins, destruction is how they end...
and the most important part.. does any of this speculation really matter?
I don' think Arcane would be an element, It would be something similar to Mana, and i don't remember the sorceress casting bolts of mana.
4th element in nature would usually be earth.
keep in mind my speculation is not in what will happen in the game, merely at as to yet unseen parts of the diablo universe's lore.
I don't by anymeans think i am so smart that i 'nailed it'. i am just tossing ideas out there for consumption in hopes that it rattles something loose in someone elses brain.

Sanctuary, High Heavens, and Burning Hells have always existed since D1
There are more than just 3 races: Vipers, Catpeople, etc
Fire, Ice, and Lightning existed in D2 also
The only people trying to keep balance in the Diablo Universe is the Necromancers. The High Heavens and Burning Hells are very simaler: Both want to conquer Sanctuary, both want to destroy the other, both don't care what gets in their way. Also, humanity was created by the union of Lilith (Daughter of Mephisto, Mother of Demons, Mother of Andariel) and Inarius (Archangel, now imprisoned in the Burning Hells) AKA offspring of both Angel and Demon
Christianity and Diablo Universe are two different concepts and should not be related anyhow except by vocabulary and naming

I believe that demons change everytime they are sent to the Abyss and come back. It would just make sense. They have to materialize into the World again...who is to say it has (or even can be) the exact same as before.
Mephisto wasn't contained within a Mortal body though. His soul was placed into a Soulstone and buried in a vault beneath the Temple of Light in Kurast. He was set free by corrupting the local priests who then shattered his Soulstone using magic, releasing his soul.
So what we see of Mephisto is actually his true form. I might be mistaken though. Check here for more information.
No...Mephisto divided up the shards of his shattered soulstone amongst his priests. His high priest took the largest of the shards and became the embodiment of Mephisto.
Ah yeah, I see that now. Always getting things mixed up, still learning on the lore so I'm still building my knowledge.

Yea...we have only seen them shifting their hosts' body to look like their true evil self (you can find this in the diablo 1 manual describing Diablo's first transformation in the Prince's body).
I will cotnradict that statment on that. One of the Prime Evils did change into their 'true' or close to true form AKA Diablo
Mephisto: Now my younger brother, it is time to assume your true form! Diablo: *Sheds mortal body* Mephisto: Arise Diablo, Lord of Terror!
Still I believe this to be him finally fully possessing the Wanderer and being able to shift the mortal's body to "assume" his true form just as he did with the Prince (detailed description of what happened in the D1 manual).
**EDIT** I went back and watched the Act 4 cinematic. Diablo doesn't "shed" the mortal's body. He just mutates it to "assume" his true form. Remember of the reasons Sanctuary became important for Heaven/Hell? Because demons/angels found that they could store up great amounts of power in humans(nephalem) on Sanctuary...Diablo would have wanted to keep this "power storage" when entering Hell...

But I don't like the idea of the evils morphed into one. I would much rather fight the 3 seperate evils all at the same time. More strategic that way.

I don't believe poison is an element. Its not a force of the earth....
